  3. Creating form • Creating form • Log into Google Docs account from docs.google.com.  Click Create New button on the top left corner of the page and select Form.

  5. Creating form • Creating form • Create form of multiple question • Go to add items , click on multiple choice. Click on question title and add the question sentence and then click to add option or other

  7. Option in creating • Option in creating • Edit question or form • After click done if you need to edit the question again click on panicle icon and reedit the question. OR you can edit your form again by opening the form again in the spreadsheet and then go to form, edit form.

  10. View response • View response • Viewing response in spreadsheet • After you open the form click on see responses, spreadsheet. Then you can open the form in a sheet.

  11. View response • View response • show summary • After you open the form click on see responses, summary. Then you can open the form as a summary.

  12. View response • View response • viewing response in Excel • After you open the form in spreadsheet click on file, download as then Excel. Then you can save the form in a excel sheet.

  13. Email the form • Email the form • After you create form, you can send it as an email, go to send and fill the box with all information. OR you can copy the link that is presented at the end of the page and paste it in any email.
