DEVELOP ideas for improvement. • Seek and discuss ideas • Explore needed resources/support • 4. AGREE on specific action plan and self-monitoring method. • Specify actions to take, including contingency plans if appropriate • Confirm needed resources/support • Confirm how and when to track progress • 5.
