Health Care Provider Stroke Education:
  • Through all of our programming we reached 1480 health care providers in the 09/10 fiscal year (see Chart #1). This is a considerably lower number than previous years for several reasons:
  • Workshops provided this year focused on skill-development and therefore accommodated fewer numbers e.g. Motivational Interviewing workshop maximum number 30.
  • Fewer Tips and Tools workshops as the LTC was occupied with the implementation of the MDS-RAI.
  • In terms of our stroke rounds videoconference (provincial and regional), it is always difficult to obtain a true estimate as they can also be accessed through live web cast.

  • Although our primary goal is health care provider education, we do capture in our database all the consumer education that occurs in SWO through our regional district team.
  • We reached approximately 1198 consumers with general stroke education including Signs and Symptoms of Stroke, what is a stroke, and blood pressure education.

  • Consists of a series of 4 sessions:
    • Cognition, Perception and Behavior
    • Feeding and Swallowing
    • Communication
    • Mobility

Patient Self-Management

A resource to help healthcare providers enhance their understanding of the various self-management approaches: individual or group that are available.

Includes a DVD with electronic copies of the tools in the resource.

A website that contains resources, and modules to help health care providers learn more about self-management. There are quizzes included in each module so you can test your knowledge. The Homeroom feature allows you to share experiences in using either motivational interviewing or self-management. You can also post questions.

  • Promote the uptake and use of Best Practices in stroke care through the use of stroke networks.
  • Created Stroke Networks in SWO:

SLP Network, Recreation Therapist Network, Social Worker Network.

  • They have an annual face to face meeting, and meet via teleconference twice a year.
