FMHS Phone App I-PHONE DOWNLOAD FROM App storeDROID: GOOGLE OR WE HAVE A SCAN FOR IT AT THE HIGH SCHOOLWhen you go to the app store or google you go to “fruita monument high school” it will bring up the app for our school. We put all important information as a push when we have events or changes. This is a great tool.

  5. REPORTING INJURIES Be sure to report any injury to your coach. Coaches will refer athletes to the Athletic Trainer. If an athlete needs to see a physician regarding an injury or illness, they must have a note from the Dr. to return to play.

  6. ROCKY MOUNTAIN ORTHOPAEDICS SPORT CLINIC EVERY MONDAY • 627 25 ½ ROAD. • 5:00 PM Mondays (after 5:30 may not be seen). • Free screening – including x-rays if needed. • Take your athlete if you are questioning an injury.

  7. IMPACT TESTING ALL ATHLETES IN THE FOLLOWING WILL PARTICIPATE IN THE IMPACT CONCUSSION TEST: Baseball, Basketball, Cheerleading, Football, Lacrosse, Soccer, Softball, Wrestling STUDENT-ATHLETES WILL BE TESTED EVERY TWO YEARS. THIS IS A NEUROCOGNITIVE TEST, NOT ACADEMIC IF A SUSPECTED CONCUSSION OCCURES STUDENTS WILL BE RETESTED AND THE RESULTS WILL BE USED AS A TOOL TO HELP MAKE AN INFORMED DECISION ON RETURN.

  8. CONCUSSION RETURN TO PLAY 5 STEP PROCESS The trainer Erin is the only one who can get athlete to return to play.

  9. RTP(return to play) Day 1: No physical activity as long there are no symptoms, for 24 hours. Day 2: Light Aerobic activity Day 3: Sport Specific Exercises Day 4: Non-Contact training drills Day 5: Full-Contact practice

ADOLESCENT ALCOHOL AND OTHER DRUG ABUSE • QUICK FACTS: • Many families struggle with teen alcohol and other drug use or misuse. • Even though it is illegal for anyone under the age of 21 to buy or use many teens are. • In a large survey Colorado ranks in the top ten states for 12 year olds. • Reason why: • to improve their moods • to receive social rewards • to reduce negative feelings • to avoid social rejection

  11. ATTENDANCE RULE • Students who have an unexcused absence will not be allowed to participate in practice or a contest on the day it occurs. If there is no practice or contest on that day, it would be enforced on the next day, Including Saturday. • Students who miss for an illness during a day are not eligible to participate unless they have attended ½ of their classes with an excuse. • Under extenuating circumstances administration can excuse an absence.

  12. TRANSPORTATION When teams are participating out side of the Valley, Athletes must ride with the team on provided transportation. Athletes riding home with a Parent must sign a release form with the head coach. If extenuating circumstance come up you must have approval from the Athletic Director. NO athlete can be given permission to ride home with anyone other than the parents.

  13. ACADEMICS FMHS will take weekly eligibility. Athletes cannot have more than one F. If ineligible for the week penalty runs the next Monday to the following Monday. Semester Eligibility, the Athlete must have passed 2.5 credits and does not have more than one F.
