PHOTO ID PROOF • PASSPORT. • PAN CARD. • VOTER’S ID. • DRIVING LICENCE. • RATION CARD WHERE PHOTO AVAILABLE. *(If ID proof is in Vernacular (Other than English), needs to be translated to English & Attested by Authorized person with seal. ID proof is a must in all cases. In all the above mentioned cases Photo must be very clear for verification)

INCOMEPROOF • Latest Income Tax assessment order / Income Tax Return. • Employer’s Certificate. • Audited Company Account’s. • Audited firm account and partnership deed. • Chartered Accountant’s Certificate with Stamp & membership Number. • Agricultural – land details & Income assessments (if agricultural income proof is in vernacular needs to be translated to English & attested by BM) . • Bank Cash – flows statement, pass – book.

  11. Mode of Payment (Yearly / Half Yearly / Quarterly ) The Payment should be made only on Demand Draft in favors of LIC of India. *Payable at – Bangalore * Cash payments are strictly prohibited. If payment is made in cash, the company is not responsible.*

  12. Swadesh Sub Agent Incentives 1.- Combination PV Incentives - • Premium Rs. 1,000 = 1PV* • 1PV = Rs. 45 • 1 Combination PV = Rs. 90 • Maximum Incentive Rs. 90,000 for 15 days. • Incentive will be calculated on 15th and last date of the month. • * ( PV = Premium Volume)

  13. 2nd Year Premium Incentives 2.- Combination PV Incentives - • Premium Rs. 1,000 = 1PV • 1 Combination PV = Rs. 15 • Maximum Incentive Rs. 15,000 for • 15 days. • Incentive will be calculated on 15th and last date of the month.

  14. 3rd Year Premium Incentives 3.- Combination PV Incentives - • Premium Rs. 1,000 1PV • 1 Combination PV = Rs. 15 • Maximum Incentive Rs. 15,000 for • 15 days. • Incentive will be calculated on 15th • and last date of the month.

  15. 4. Life Incentive • Premium Rs. 1,000 =1PV • 1 Combination PV = Rs. 10 • Maximum Incentive Rs. 10,000 for 15 days. • Incentive will be calculated on 15thand last • date of the month. • Policy Premium will be paid 4th Year onwards • you will be get life incentives.

  32. Rules and Regulations # All incentives and appreciations etc..
