LTIFR = Lost Time Injury Frequency Rate (includes LTI’s only; no longer includes fatalities) CIFR = Classified Injury Frequency Rate (Fatality + LTI + RWI) TRIFR = Total recordable Injury Frequency Rate (Fatality + LTI + RWI + MTI)- Splitting the NLTI this year from Jan, hence the graph started reading from Jan AIFR = All Injury Frequency Rate (Fatality + LTI + RWI + MTI + FAI)

  6. Jwaneng mine officially launched the Near Hit reporting system on the 26th Oct, 2011. A database has been developed for capturing Near Hits A significant number of employees have been trained on Near Hit reporting, data capturing and given access to the database Management Plans-Near Hits

  7. Operational control: Developed contractor SHE file which is based on the OHSAS 18001 requirements. This file has been discussed with the contractors. We will pilot the file for the next two months and implementation is planned to commence in January 2012. Visible Felt Leadership (VFL) is planned to be led by Excowith line management. Union members to be included as part of the process starting in November 2011. FRCG: Implementation gap assessment was conducted by a consultant. This will contribute to the scope of work to be conducted to meet full compliance by end of 2012. Management Plans
