Methodology • Review key studies between 2001-2011 from Medline • Keywords: family planning services, family planning policy, contraception, birth intervals, breastfeeding, exclusive breastfeeding (EBF), maternal nutrition, birth outcomes, pregnancy outcomes, pregnancy intervals and short birth interval • Experts in the fields of FP and nutrition reviewed

The effects of pregnancy spacing on infant and child mortality in Matlab, Bangladesh: How they vary by the type of pregnancy outcome that began the interval. Population Studies 62(2): 131–154.   • Assessed effects of duration of intervals on infant and child mortality • 125,720 live births - Bangladesh • Controlling for other correlates of infant and child mortality, found that shorter intervals are associated with higher mortality • Many relationships found were consistent with the maternal depletion hypothesis, and some with sibling competition 4

  5. Dewey KG, Cohen RJ. 2007. Does birth spacing affect maternal or child nutritional status? A systematic literature review. Maternal and Child Nutrition 3(3): 151–173. • Search of relevant papers since 1966 – 34 met criteria • Child nutrition studies with longer birth interval are associated with a lower risk of malnutrition in some populations, but not all • Reduction in stunting associated with a previous birth interval ≥ 36 months ranged from 10% to 50% • May be due to residual confounding, BF and/or maternal height • Maternal anthropometric outcomes yielded mixed results - further research needed 5

Maternal and Child Nutrition 5(1): 49–63.  • Examines relationship between birth spacing & childhood undernutrition (stunting & underweight) • El Salvador • Birth intervals of less than 24 months and intervals of 24–35 months significantly increase the odds of stunting • Policy and program implications: more effective use of health services/outreach programs on FP, BF and well child care 6

The Journal of Nutrition; May; 133, 5S. pg.S1732. • Previously assumed fetus functioned as parasite and withdrew nutritional needs from maternal tissues • Studies demonstrate that if the maternal nutrient supply is inadequate a state of biological competition exists • In severe deficiencies maternal nutrition is given preference; in a marginal state the fetal compartment is favored • Supplementation with food and micronutrients during interpregnancy period may improve outcomes 7

Further evidence of the effects of preceding birth intervals on neonatal, infant, and under-five-years mortality and nutritional status in developing countries: Evidence from the Demographic and Health Surveys. DHS Working Papers, Demographic and Health Research (41). • Effects of birth-to-pregnancy interval for infant/child mortality • Data from 52 DHS surveys from 2000-2005 (1,123,454 births) • Risk of mortality is highest for very short intervals (<12 months birth-to-pregnancy) but relatively few children conceived at such intervals (14 %) • Children conceived between 12 and 35 months (42%) are at greatest risk • If all women would wait at least 24 months to conceive again, under-five deaths would fall by 13 percent. • The effect of waiting 36 months to conceive again would avoid 25 percent of under-five deaths 8

  10. Becker S, Ahmed S, 2001. Dynamics of contraceptive use and breastfeeding during the postpartum period in Peru and Indonesia. Population Studies (55): 165–179. • Analysis of DHS data from Peru & Indonesia • BF women < likely to have resumed sexual intercourse in the early PP in both countries than non-BF women • FP uptake highest after resumption of menses • 10% subsequent pregnancies occurred before menses • Results emphasize importance of integrating BF counseling and FP services in programs serving postpartum women

  11. Jakobsen MS, et al. 2003. Termination of breastfeeding after 12 months of age due to a new pregnancy and other causes is associated with increased mortality in Guinea-Bissau. International Journal of Epidemiology; 32:92–96. • Assessment of BF and child health in Guinea-Bissau • Investigated impact of mother’s reason for weaning on child mortality (1423 children who terminated BF after 12 months were followed) • 62% were weaned because they were ‘healthy’. Compared with the ‘healthy’ children, all other causes of weaning were associated with a higher mortality (MR = 2.97) • For the 237 children weaned due to a new pregnancy the mortality ratio was highest (MR = 3.25). • Weaning due to new pregnancy of the mother is associated with higher mortality 11

Contraception (77): 350–354. • Contraceptive practice & unintended pregnancy - Egypt • > 95% of women breastfed their child before current pregnancy • 25.3% conceived while breastfeeding. (Conception during first 6 months PP = 4.4%, before resumption of menstruation = 15.1%, while exclusively or almost exclusively breastfeeding = 28.1%) • Only 10 pregnancies (1.5%) occurred when all LAM prerequisites were present • 29% of pregnancies conceived during breastfeeding were unintended • Pregnancy during breastfeeding is common and often unintended. Great potential for LAM, but must be properly taught, and women must be encouraged to use effective contraception after LAM expires 12

  14. Gebreselassie T, Rutstein SO, Mishra V. 2005. Contraceptive use, breastfeeding, amenorrhea and abstinence during the postpartum period: An analysis of four countries. DHS Analytical Studies (14). • Looked at DHS from Kenya, Indonesia, DR and Peru for time to first use of FP after a birth, duration of breastfeeding, and postpartum amenorrhea • 20-40% don’t start FP when they are at risk for another pregnancy • Prenatal programs should include information on BF and PP contraception, as well as during the PP check before discharge and during PP home visits. 14

  15. Tilley IB, et al. 2009. Breastfeeding and contraception use among women with unplanned pregnancies less than 2 years after delivery. International Journal of Gynecology and Obstetrics (105): 127–130. • Examined BF and contraceptive use after LAM criteria were no longer met (Egypt) • 81.5% of women with unplanned pregnancies were breastfeeding at conception • Of these women, 36.3% had used a method of contraception other than LAM compared with 60.5% of women who had weaned • Among the breastfeeding women, 61.2% failed to use contraception because they believed breastfeeding would prevent pregnancy • Breastfeeding women with unplanned pregnancies were less likely to have used contraception than women who had weaned 15

  17. IIIf, et al. 2005. Early exclusive breastfeeding reduces the risk of postnatal HIV-1 transmission and increases HIV-free survival. AIDS 19(7), 699-708.    • Trial of PP Vitamin A supplementation collected info on infant feeding practices, infant infections and deaths • Compared with EBF, early mixed breastfeeding was associated with a 4.03, 3.79, and 2.60 greater risk of Post-natal transmission (PNT) at 6, 12, and 18 months • Compared with EBF, predominant breastfeeding was associated with a 2.63, 2.69 and 1.61 trend towards greater PNT risk at 6, 12, and 18 months • Conclusion: EBF may substantially reduce breastfeeding associated HIV transmission. 17

March 30, 2011. • Best Practices: • Several studies showed LAM users are more likely to use FP • Working at the community level with CHWs, using counseling tools and job aids, and simple and culturally appropriate BCC materials • Barriers: • Nutritionists & FP practitioners not trained in the other’s perspective • Different services often exist in silos within health systems • Gaps in Research • Little literature on FP-nutrition integration - programmatic evidence is lacking • Lack of an agreed-upon research agenda • Presentation of Cochrane Review 18
