  1. Children’s Hospital of Chicago Research Center Leadership Meeting Cayuse IACUC FAQs September 20, 2012

  2. Can my administrative assistant or some other staff member enter the information into Cayuse? Yes, however, many of the questions require scientific understanding of the procedures and experiments being conducted under the protocol. Please be certain to give clear direction and carefully review your submission to ensure timely review by the IACUC.

  3. Why can’t I save a partially completed section or skip a section? • You must complete the introductory page (AKA addendum checklist) first. The addendum checklist populates the sections needed to complete your application. • Sections can then be completed in any order. However, a section MUST BE COMPLETED before it can be saved. • This ensures all questions in a section are completed before submission.

  4. What is the rationale for moving active protocols into the Cayuse system before the date of their next de novo review? • The goal is to transition all studies into Cayuse within the next year. Consistency in recordkeeping is necessary for reporting to regulatory agencies. • The deadline assigned for each protocol is based on the anniversary of the original approval date in order to spread out the process over twelve months. • These submissions will not be “re-reviewed” by the fully convened IACUC and will retain their original review date, review period, and review cycle (3 years for non-USDA species; 1 year for USDA species). • Once all existing protocols are entered into Cayuse, the process for submitting and reviewing continuations, amendments, and renewals will be much more efficient for the PI and the IACUC. • **Submission deadlines can be flexible if the PI works with the IACUC office particularly for those PIs with multiple submissions.

  5. Why is the Program Leader/Division Head listed in the personnel section of Cayuse IACUC? • In order to review and approve the protocol, the Program Leader/Division Head must have a role in Cayuse. • The role given is that of “Program Leader/Division Head” and the assignment for this role is “Review & Approve Only.” • This role is differentiated from others actively involved in the protocol (i.e., animal handlers, etc.).

  6. Why does the program leader/division head need to copy and paste the approval statement into Cayuse? • The Cayuse IACUC module currently does not have a routing feature (like SP module) and Cayuse is working towards a resolution. • In the meantime, to streamline this process, the approval statement is now listed in the header of Section. 12. the Program Leader/Division Head needs to type “Reviewed and Approved”, their name and date in Section 12. • When the Cayuse protocol submission is complete, the PI will email the Program Leader/Division Head, who may then log in, review, and approve the submission. • *Note that we have eliminated the submission “copy and paste” requirement in response to faculty feedback.
