  7. Pre/Post Surveys - The Basics • Anonymous Pre and Post Test • Administered before 1st lesson and after the final lesson in a series of lessons • Convenience Sample • Administer with a series of lessons as desired • The survey may be read allowed • Participants should work independently

  12. Administering the Survey • Methods • Paper • ARS • Web (Illume) directly if technology available • Ok to clarify pronunciation of words or define/ translate words • Do not help with selecting answers

  13. Introduction I’m _________ (your name), a representative of the University of Arizona Nutrition Network, and I would like to invite you to complete a survey. We want to learn more about the eating and physical activity habits of people living in Arizona. This survey is completely voluntary and your name will not be used. You may skip any questions you are not comfortable answering or stop taking the survey at any time. It will take approximately 15-20 minutes to complete the survey. Does anyone have any questions before we begin?

  14. Administering the Survey • Explain that the pictures are examples only (pre/post only) • Emphasize the importance of maintaining a neutral tone and confidentiality to any individuals assisting

  15. After Administering the Survey • Return the completed surveys with the coversheet to the UANN Study Coordinating Center • Use pre-paid, pre-addressed envelopes monthly • OR…upload ARS data to ftp upon return to office (24hrs) • Provide feedback to Stephanie Martinez • shm1@email.arizona.edu
