  1. 高風險家庭通報方式一 進入內政部「關懷e起來」網站線上通報 ＞步驟1：進入網址:http://ecare.moi.gov.tw 選擇【線上通報兒少保/高風險案件】

  2. ＞步驟2：通報人身分:請選擇【責任通報】 備註： 若選擇「一般民眾」通報，只能選擇【兒少保護個案（表1）】

  3. ＞步驟3：填寫個案基本資料 個案類型:請選擇【高風險家庭(表2) 】 網址:http://ecare.moi.gov.tw

  4. ＞步驟4：簡述案情並說明已提供之服務項目，＞步驟4：簡述案情並說明已提供之服務項目， 且再次檢視所填內容後送出通報表 網址:http://ecare.moi.gov.tw

  5. 通報方式二：傳真通報 ◎請填妥兒童少年保護及高風險家庭通報表 (100年1月中央修訂版) 傳真至新北市政府高風險家庭服務管理中心 傳真通報:8968-2381 通報方式三：電話通報 ◎為方便一般民眾通報及諮詢，故設置電話 通報專線:8968-2380

  6. 填寫通報表注意事項 (1)儘可能清楚案主及其家人的基本資料及連絡方式。 (2)案情應以近期最新發展為主，若僅是「聽說」，則煩 請再次確認目前實際狀況。 (3)紀錄清楚通報的日期、時間、方式、通報人的名字、 聯絡方式及所瞭解之案情內容(案家問題)。 (4)請注意保密原則（請以密件處理，並注意維護民眾之秘密及 隱私，不得洩漏或公開，對於通報人之身分資料應予以保密，以維謢個 案本人及相關人員隱私）
