  6. Insurance WE OFFER SUPERBILL AND REIMBURSIFY PAYMENT POLICIES: All payments are to be made, at the time of service. Should your insurance company deny benefits or fail to pay for any reason, you are responsible to pay the balance due. Please note: A pre-certification is NOT a guarantee of benefit payment. CANCELLATION POLICIES: To avoid a cancellation fee, cancellations must be made at least 24 hrs prior to your scheduled appointment time. Cancellations made after 24 hrs and No Shows are subject to the full private pay fee for the time scheduled. FEE SCHEDULE: Private Pay/Superbill Initial Visit (90 minutes) – $150.00 Single Session (50 minutes) – $100.00 Double Session (100 minutes) – $200.00 Couple Session (50 minutes) -$200.00 NOTE: SLIDING SCALE FEES MAY BE AVAILABLE WITH INCOME VERIFICATION.
