Portfolio Evaluation Forms • Fill in your information at the top of the FACT Mid Year Advisement Checklist • Insert the form into the front of your Portfolio Remove Documents • Take your completed IIP and Essential Components from your Portfolio Hand off Portfolio • Submit your Portfolio to a reviewer
