Findings: More than did performance improve? yes or no Multiple sites allow for examining change in varying conditions and settings • After QA intervention, full-time practitioners improved more than part-time practitioners • Female practitioners were more likely to perform cancer screening among women than male practitioners • Individual practitioner performance varies from patient-to-patient, from guideline-to-guideline • Practice leaders who participated in QA influenced performance among others in the practice • Practitioners in larger health centers performed better and improved more than those in smaller centers

  10. Overview of the Project An evaluation of the effects of enhanced training of community health workers (CHWs) to reduce disparities in care in a low-income, racially and ethnically diverse population served by 12 community health centers (CHCs)

  13. Community Health Patients • Nearly 40% are insured through Medicaid • Approximately 40% are low-income uninsured and underinsured • More than 60% belong to an ethnic, racial or linguistic minority group • Fewer than 20% are insured by Medicare or commercial insurance

  14. An Intervention with Community Health Workers • Curriculum developed for health worker training related to the Care Model and diabetes self-management • Reviewed and customized for divergent cultural, linguistic, and ethnic groups • Training sessions equal to 45 hours

  20. Unexpected Events • Health center needed to drop out • Health workers were often pulled in other directions • Monitoring was more necessary than planned • Data collection system, not build for research

Lessons Learned • PECS was not designed for research • Disease management registry • Systems on site (e.g., EHRs) are not designed for research • Training was key, needed continuing education • CHWs were part-time on the project • Valuable assets of health centers • Needed to monitor activity on the study • Focus groups: Inadequate preparation of health center, i.e., many didn’t know the study was there • Encounter data was essential

The Takeaway • Employing CHC staff was necessary to get buy-in, yet hard to manage • Need to do a better job of working with sites, i.e., meet with staff, work with management • CHWs need more extensive training in how to work with patients • Structured encounters was the bare minimum • Planned activities was helpful, but need specifics for ongoing visits • Test your data entry beyond “getting a report” • Need better planning for unexpected outcomes

Essential characteristics of motivational interviewing include: • Expressing empathy through reflective listening. • Noting discrepancies between current and desired behavior. • Avoiding argumentation and rolling with resistance. • Encourage the consumer's belief that he or she has the ability to change. • Communicating respect for and acceptance of people and their feelings. • Establishing a nonjudgmental, collaborative relationship. • Being a supportive and knowledgeable consultant. • Complimenting rather than denigrating. • Listening rather than telling. • Gently persuading, with the understanding that change is up to the person. • Providing support throughout the process. Applying Lessons Learned from FindingAnswers CHW Study • Manageable number of sites • Allow for more contact with staff, including providers • More training to work with patients, specifically in motivational interviewing ,was inadequate • Build in monitoring, supervision • Use better technology for data collection and management

  33. Guiding Evaluation of Interventions “Researchers and practitioners are typically most interested in the outcomes or results of their study or program. If they have thoughtfully designed the intervention, it is likely that theory has guided the effort. Conceptual and logic models can be used to depict how intervention activities or “inputs” are expected to produce desired outcomes by way of changes in intermediate factors that are often theoretically linked. For example, changes in colorectal cancer screening behavior are more likely to occur when there are subsequent changes in knowledge, attitudes, perceived susceptibility to colon cancer, and reduced barriers to obtaining screening”.   37. Lessons Learned • Use Logic Models to plan evaluation • Building Data Collection into the Fabric • You may not know what you need to measure (e.g., hours, time, etc.). Therefore, within reason, try to measure comprehensively • Use Enrollment charts (e.g., Targeted Enrollment/Week and Actual Enrollment) • Encounter forms are nice but a cloud-based system is better, e.g, REDCap

How to Infuse Fidelity Spillane et al. recommend many varied treatment fidelity procedures to improve the reliability and validity of research including: • Use of manuals and standardized guidelines for intervention providers. • Researcher observations through video/audiotaped observation of the intervention (e.g. a consultation). • Self-report evaluation forms/checklists completed after consultations by those delivering the intervention. • Quality assurance checks completed after the consultations by the researcher. • Researcher field notes regarding the study intervention (e.g. requests for additional assistance with intervention implementation and records of all phone and face-to-face contact between interventionists and researchers). • Case conferences that provide a forum for discussion of the intervention. • Questionnaire surveys/focus groups/interviews with intervention providers and recipients. • Monitoring and testing study participants’ acquisition of treatment skills.
