Voluntary Life • Affordable way to add extra life insurance protection for your family. • Benefit is portable and convertible, should you leave your employment. • Evidence of insurability will be required for any employee applying after 30 days of their date of hire, and/or above the guaranteed issue amounts. If you want to apply for coverage or increase the amount of your current coverage, please complete this form and submit to your plan administrator. • Rates are based on employee age; there is no distinction between smoker and non-smoker. • See Principal summary for rates & plan information.

  10. Find a Provider Medical Providers: 1). Go to www.bcbsga.com 2). Click on “Find a Doctor” 3). Either enter in your member ID number, or select “Employer Group Plan” and enter your zip code 4). Select the network – Point of Service Plans; choose “Blue Open Access POS” Dental and Vision Providers: 1). Go to https://principal.com 2). Dental: Select your search parameter enter your state and select “Principal Plan PPO” 4). Vision: No Network. You can see any vision provider of your choice.
