Benefits Open Enrollment • Open Enrollment begins Monday, November 11 through Monday, December 2, 2013 • The Dayton Board of Education still pays 85% while employees pay only 15% of the total benefit costs • Changes become effective January 1, 2014 • New rates reflect a 7% increase in Medical • There will be a slight increase for Dental

Will I receive a paper copy of the benefits guide? • All forms will be posted online at: • www.benefitsnapshot.com/daytonpublicschools • Employees without regular access to a computer and those on Leave of Absence should call the Office of Human Resources and request a paper copy of the forms they need
Who can I cover on my Medical and Dental Plans? • Yourself • Your spouse or registered same-sex domestic partner • Your dependent children up to age 26 for Medical • Your dependent children up to age 23 for Dental • For dependent eligibility definition, visit our benefits website: • www.benefitsnapshot.com/daytonpublicschools
Will I receive new Medical and Dental ID cards? • Members will receive a new Medical ID card from United Healthcare • Members will NOT receive Dental ID cards from Delta Dental • Your coverage is identified by your social security number • Replacement cards for Medical can be obtained by contacting United Healthcare directly
Will I receive a new FSA debit card? • No - current participants will not receive new cards • You will have your new 2014 health care FSA election loaded on your current debit card • Please hold on to your FSA card after you use up the account balance • New health care FSA enrollees will receive new FSA debit cards • Replacement cards can be obtained from FlexBank
