Time Out Included All 5 Required Elements (patient, procedure, site/side, position, & specialty equip) Note: Oct 07 results - Patient and procedure were included for 100% of cases; Site* missed for 18% cases; position 13%, & specialty equip/implants/xrays 12% Staff feel in some cases procedure indicates site/side; e.g. hysterectomy
