  3. AFIX visits in Private Provider offices • AFIX offered since 1995 • Voluntary by the practice • Michigan uses “classic CASA” • 2002 Goal:150 private provider sites Michigan Department of Community Health - Bureau of Epidemiology

  4. How Did We Get Here? • Requested an increase in grant funding to hire additional staff • Marketing, Marketing, Marketing • What is in it for them? Michigan Department of Community Health - Bureau of Epidemiology

  5. How can we reach the goal? • MDCH AFIX staff • 1 Coordinator • 3 Assessment Specialists • 1 Administrative Assistant • Marketing providers enrolled in VFC and the Michigan Childhood Immunization Registry (MCIR) Michigan Department of Community Health - Bureau of Epidemiology

The AFIX Process Provider Office calls MDCH Chart Review & Feedback scheduled Provider office generates list of 19-36 month-olds Provider office pulls charts Maximum 200 MDCH does Chart review at Provider office CASA reports generated and analyzed Quality assurance check conducted with the MCIR Feedback meeting at provider office 1 hour presentation Evaluations on process and content Referrals made for additional follow up Letter to practice Requesting another AFIX Michigan Department of Community Health - Bureau of Epidemiology
