  2. Contacted by email, phone and lab visits to schedule a date/time • Check cyclotron calendar and propose a time and mark the cyclotron calendar • Tech emails CCIR #, patient info along with time/date to registry • Registry will register patient, order visit, schedule visit; Mark date/time on Google calendar for PET room

  3. Scan Participant (by technician unless indicated otherwise) • Coordinator on behalf of participant signs in LOG • Receptionist confirms appointment in CS and change status to “Arrived” • Receptionist obtains first/last pages of consent and validates consent in CS • Receptionist prints out Procedure Assessment Form and gives it to coordinator • Receptionist changes the status to “Proceed” in CS • Tech verifies participant identity • Update randomized name and patient id in CS and change status to “Started” in CS • Perform scan • Record information in printed PAF and electronic LOG in CS

  4. Scan Participant (continued) • Change status in CS to “Started” • Inject contrast • Perform scan • Change status in CS to “Ex Done” • Data distribution • Billing
