Submit a softcopy (e-mail to the VP for Academics) or hardcopy (to the VP for Academics’ pigeonhole in the tambayan of the accomplished request slip at least 3 days (for groups less than 7 people) or 5 days (for 7 or more people) before the target tutorial date. Secure Tutorial Request Form (ACheS Bulletin Board or online: academaches.wordpress.com) academACheS Services Submit accomplished form to VP Academics If approved, TUTORIAL SESSION Wait for confirmation via text or email.
