  4. TSB Investigations • 4000+ occurrences reported annually • All occurrences tracked in database • Marine Branch: 544 reportable marine incidents and accidents in 2011 (20 fatalities) • 2011: 10 investigations started, 14 safety letters, 2 Board recommendations

  5. Board Responsibilities Safety-critical information is shared ASAP “Designated reviewers” comment on early drafts and provide feedback The Board approves all investigation reports Recommendations for difficult, systemic issues Safety Information Letters Safety Advisories

Watchlist 2012 Marine Safety Management Systems Loss of Life on Fishing Vessels Landing Accidents and Runway Overruns Air Safety Management Systems Risk of Collisions on Runways Collisions with Land and Water Passenger Trains Colliding with Vehicles On-Board Video and Voice Recorders Following Signal Indications

Marine SMS Issue: TC does not always provide effective oversight of marine transportation companies transitioning to safety management systems, while some companies are not even required to have one. Solution:TC should require all commercial vessels to have SMS, and all SMS should be certified and audited.

  8. Loss of Life on Fishing Vessels Issue: The number of accidents involving loss of life on fishing vessels remains too high. Solution:Concerted and coordinated action is required by leaders in the fishing community to implement a safety culture approach to fishing operations, recognizing the interaction of safety deficiencies.
