Objectives • Integrate data collection with clinical documentation to minimize paperwork burden • Develop strategies for improving outcome data, especially for a chronic, relapsing population with a disease management approach • Explore uses of the data that are beneficial to direct service providers to ensure the quality of the data collection

  7. Overarching Outcome Measures:MH, SA, MR Community Consumer Submission (CCS) • Stability of living arrangement • Employment/ “meaningful, productive activity” (adults only) Data Collection To Be Determined (TBD) • Managing illness/ condition (MH, SA only)

  8. Additional Outcome Measures: SA CCS • Reduction in frequency of alcohol use • Reduction in frequency of drug use • Reduction in # of arrests • Improvement in employment status Italics = Overarching Outcome 4

MR Outcome Measures MR Survey • Increase in number of family members/guardians helping develop ISP • Increase in number of family members/guardians who felt that staff respected the family’s choices and opinions • Increase in number of family members/guardians who felt CM gave them assistance when asked

  13. MR Outcome Measures • Increase in number of family members/guardians who felt their person had access to supports and services when needed • Increase in number of family members/guardians who felt that the day or employment program was healthy and safe

MR Outcome Measures • Increase in number of family members/guardians who felt that frequent changes in staff who work directly with the person has not been a problem • Increase in number of family members/guardians who felt that the person received assistance right away in an emergency or crisis
