  1. Promotion to Professional Sequence Application Instructional Guide 2014 Please read entire document carefully. Most of your questions will be answered here.

  2. Who should apply? • Any undergraduate student who is an ES intended major, currently enrolled or accepted to UB who will have successfully completed all prerequisite courses with a prerequisite GPA> 2.5 by the end of Spring 2014, who wishes to enroll in the upper division exercise science courses. • Prerequisite courses can be found here: http://sphhp.buffalo.edu/exercise-and-nutrition-sciences/education/exercise-science-bs/prerequisites.html • Variety of degree options students can follow  • BS, BS/MS, BS/MPH, Pre-PT(DPT)

  3. Reapplying to the Program • How many times can I apply? • You can apply to the program up to two times • If I was dismissed from the upper division ES program, can I reapply? • No, if you were dismissed from the upper division ES program you are not eligible to reapply

  4. When can I apply? The application system opens January 1st and closes the February 7, 2014. Late applications will only be considered if space is available.

  6. Learn to Love HUB & yourAcademic Advising Report (AAR) • You will need to refer to your AAR often and will need to pull information from it for your application. • To access your AAR, start by logging into MyUB • Click on HUB Student Center • Under Academics, choose Academic Requirements from the drop down box on the left side of the screen • This will open your AAR • For more information on how to use HUB, please visit www.buffalo.edu/hub/ • If you are having problems with HUB, click the Faculty & Staff Help button on the right-hand side of the page

How do I complete the application? ***Be sure you application is filled out completely before submitting!! ***Partially submitted applications will not be evaluated– this includes transcripts and Proof of Enrollment Step 1- Demographic Information Step 2- Program Selection Step 3- Prerequisite Courses Step 4- General Education Courses Step 5- Student Notes Step 6- Student Appeals Step 7- Privacy Statement Step 8- Submit Application Step 9- Official Transcripts Step 10- Proof of Enrollment

  8. 1- Demographic Information Complete personal information. Please remember what phone number, email or address you have listed. If any of these change during the application process, WE NEED TO KNOW!

  9. 2- Program Selection • Choose your intended track: • General Exercise Science: General 4 year BS degree • Pre-Physical Therapy: applying to DPT next year before completing BS in ES • Prerequisite GPA > 3.0 • If you do not meet the minimum prerequisite GPA you can still select the Pre-PT concentration, however if you are accepted you will initially be placed in the General ES Concentration • BS/ES and DPT: complete 4 year BS before applying to DPT program • BS/MS in Exercise Science & Nutrition: 5-year program, BS in ES and MS in Nutrition Science • Prerequisite GPA > 3.0 • BS/MPH in Exercise Science & Epidemiology: 5 year program, BS in ES and MPH in Epidemiology • Prerequisite GPA > 3.0 **This can change once you are accepted into the program and meet with your advisor. This is not set in stone from the application. **

  10. 3- Prerequisite Courses • Enter course number, institution, semester & year, credit hours, grade. • Course number: Use UB course number. For example, if you took CHEM111 @ different institution but it articulated as CHE 101 at UB, use CHE 101. • Institution: List actual institution where you completed the course. • Semester & Year: Enter semester and year you completed course. • Credit Hours: Enter credit hours you received for this course. • Be sure to use what your AAR reports for credits and grades. Use the following slides as examples of common situations.

USE YOUR AAR! • On your AAR, you will see a section titled Exercise Science BS Acceptance Criteria, use this to fill out the application. • If there is nothing listed under it, click on the green arrow to the left to open the section up. Same for individual course requirements. • If your course is not listed here, it is most likely listed under Courses History section. Use that section to input grades.

  12. A student completed MTH121 here at UB: BASIC EXAMPLE • A student completed MTH200 for 3 credits at “Jane Doe College” which is an equivalent course to MTH121 at UB:

Labs Incorporated into Class Grade & Credit If your lab grade and credit are incorporated into the same listing on your AAR, please put the same grade in the lab section as you earned in the lecture and enter “0” for credit hours For example, if you took CHE 101 at UB your AAR will only report a grade for Chemistry lecture. Please enter as follows:

  14. Physics Enter grades for PHY 101, 151, 102 and 152

  15. Physics If you have completed PHY107, 108 and 158 sequence, please report the three courses and grades you received as follows: This is an approved sequence for our prerequisite courses.

  16. Anatomy If you took ES 207 you will enter as follows: If you took an equivalent course where you received a separate grade and credit for the lab please indicate this by entering all information.

  17. Anatomy If you have completed a 2-semester biology sequence for science majors to fulfill your anatomy requirement, please report one course as the anatomy course and one as the anatomy lab. Enter the class, credit and grade as it is stated on your AAR.

  18. Anatomy If you have completed the APY 345/346 sequence to fulfill your anatomy requirement, please report one course as the anatomy course and one as the anatomy lab. Enter the class, credit and grade as it is stated on your AAR.

  19. Combined Anatomy & Physiology Course If you took a combined course for Anatomy and Physiology outside of UB, your AAR should show a grade such as MB+. This means you received an averaged B+ grade in this course. You would report B+ for both courses on your application. If you have only completed one course in the combined sequence, unfortunately this will NOT count for either of the combined courses.

  20. Statistics If you completed STA119*, it will list it separately on your AAR as a 3 credit lecture and a 1 credit recitation. If you completed a course equivalent to STA 119 that does not include a separate lecture and recitation, list these on the first line with 3 credits, and second line with 0 credits and same grade. * or equivalent

  21. Entering Grades • Please triple-check this. We will confirm them. • Please be honest. Your application may not be accepted if grades are incorrectly reported. • Choose the appropriate letter grade from the drop down box.

Entering Grades • AP Credit • Course Number- Enter UB course # as it shows on your AAR • Institution- Enter the name of your high school • Grades- select P3 , P4, P5 depending on what you earned on the exam • Current Enrollment • If you are completing the course this spring, please use “E” for enrolled.

  23. Entering Grades • Retaking Courses: • As stated in the UG Catalog, all 2nd attempt grades will count as your official grade, regardless if it is better or worse. This is what you will enter on your application. • If you received an “F” in a course but will be retaking it this Spring  the “F” must still be reported and will be calculated in the prerequisite GPA. At this point you do not have a 2nd attempt grade. • Repeat Policy: http://undergrad-catalog.buffalo.edu/policies/grading/repeat.shtml • 2nd attempt grades at another institution (after passing the course at UB) will not be reflected in your GPA or give you a grade on your transcript

  24. Entering Grades • Higher Grade in Higher Level Course: • If you earn a higher grade in a higher level course in the same dept you may report the better grade. However, if this course is also an ES elective you can not use it for both a prerequisite requirement and an elective requirement. • For example, if you earn a “C” in CHE 101, and an “A” in CHE 201, you may enter CHE 201 as your prerequisite course and grade. But you will NOT be able to use this course as an ES elective requirement in the future. • Note: You cannot use CHE 101 and CHE 201 – you must use the courses that would satisfy the sequence (i.e. CHE 101 or 201 and CHE 102 or 202)

  25. Missing Prerequisite Courses If you are missing prerequisite courses, leave the field blank. You MUST list any course you will not have completed in Spring 2014 and indicate why in the Student Appeals box at the end of the application. Only in exceptional cases will a student be accepted with missing prerequisites.

  26. Articulated Courses • What does this mean? • Articulation refers to the process of comparing the content of courses that are transferred between educational institutions and deciding if they are equivalent. • A course MUST be articulated through UB to be used as a prerequisite on your PPS Application. • Any course that has been articulated by UB should show up on your AAR with a UB equivalent course number. You will use UB course numbers when filling out this application.

  27. Transfer Course Articulation • If a course has ALREADY been articulated by UB, it will show up on your AAR as the UB Course Number • For example, if you took PSYC 120 at a previous school and it shows up on your AAR as the UB equivalent- PSY 101, it HAS BEEN articulated • Use PSY 101 on your application • If your course has not been articulated it will not show up as a UB course number, the course will have an assigned transfer number such as "TR0005” or an unassigned course # such as “PSY 006” • If you plan to use such a course on your PPS Application, you will need to initiate the articulation processfor that course and send us proof that you took the course by February 7th (transcripts, schedule, etc.). • For all courses that have NOT articulated you must proceed accordingly ASAP

  28. Transfer Course Articulation: Already Admitted UB Students • If you have already completed a semester or more at UB and plan to use courses on your PPS Application that have NOT been articulated through UB yet, you need to follow the instructions on the Transfer Articulation Services website: http://www.taurus.buffalo.edu/form.shtml. • Please understand that articulation of unevaluated courses will be determined through the specific department not through ENS (unless it is an ES or NTR course). • For example, if you have an unevaluated chemistry course from a different institution, you will have to fill out an articulation form found on the website above and directly contact the chemistry department for course articulation.

  29. Transfer Course Articulation: Incoming Transfer Students • Send an official transcript to the Office of Undergraduate Admissions, 12 Capen Hall, University at Buffalo, Buffalo, NY 14260 if not sent already. • Next, you will need to mail, fax or email the following information to Transfer Articulation Services: • A catalog description of each transfer course and its prerequisites • The course syllabus for each transfer course • The transfer institution’s name for each transfer course • The UB requirement you think each course may work toward • Your name and 8-digit UB person number Mail: University at Buffalo, Transfer Articulation Services, MFAC 316, Buffalo, NY 14261-0026 Fax: 716-645-2288 Email: taurus@buffalo.edu • You must initiate this process as soon as possible. The review process will take a minimum of three weeks; therefore we recommend this process begin at least 3 weeks prior to the PPS Application Deadline. • If you know that the process will go beyond the application deadline, please let us know and send us proof that you took the course (transcript, schedule, etc.).

  30. Transfer Course Articulation: Non-Required Courses • If you have additional courses on your AAR that have not been articulated and are not required for ES, you do NOT have to go through the articulation process. • For example if you took Intro to Dance at a different institution and it shows up as TR002, you do NOT have to go through the articulation process if you will not use this for a requirement.

  31. 4- General Education Courses • On you AAR, take a look at the GENERAL EDUCATION section. • For each course requirement on the application, check either Satisfied, Not Satisfied or Waived according to what you AAR indicates. • Students are encouraged, but NOT required to have all General Education Requirements completed by the time they apply to the PPS.

  32. 5- Student Notes • Please list any additional information you would like the selection committee to know here. • Example #1: you will be moving in the middle of the spring semester. Please indicate this here and new address/contact information we can reach you at. • Example #2: You will be studying abroad during spring semester. Again, please indicate how we can reach you best during this time.

  33. 6- Student Appeals • If any prerequisite courses will not be completed by the end of Spring Semester 2014, you need to  • List each course you will be missing • Indicate WHY you will not have it completed • Only under exceptional circumstances will a student who has missing prerequisites be accepted into the upper division.

  34. 7- Privacy Statement Failure to provide required information and accurate information may prevent your application from being processed. We double check your grades!! Be honest!!

  35. 8- SUBMIT! Be sure your application is filled out completely before submitting! Triple Check! Partially submitted applications will not be evaluated. If your application is incomplete you will only be eligible for the alternate list so be sure to submit the completed application and any supplemental materials (i.e. transcripts, course schedules) by Feb 7! You may save your data and come back to it at a different time, but REMEMBER to officially submit once it is complete. *** Saved but un-submitted applications will not be considered ***

  36. 9- Official Transcripts • If you are currently a UB student and have completed at least ONE full semester at UB and have had all of your official transcripts from previous institutions sent to the Office of Admissions, you do not need to do anything more. • If you are an incoming transfer student for Spring 2014 or Fall 2014  please send an official transcript to both (if you have not done so already): • Pedro Sotelo-Peryea, Dept of Exercise and Nutrition Sciences, 211 Kimball Tower, UB, Buffalo, NY 14214-3079 • Office of Admissions, 12 Capen Hall, Buffalo, NY 14260-1660

  37. 10- Proof of Enrollment • If you are taking any prerequisite course outside of UB this Spring Semester and will be reporting it on your PPS application, you need to supply us with documented proof that you are actually enrolled in the course at the given institution. • Proof needs to be provided by February 7th. • The application will be considered incomplete if proof is not submitted by the due date. • Proof of enrollment should be submitted to: • Pedro Sotelo-Peryea, Dept of Exercise and Nutrition Sciences, 211 Kimball Tower, UB, Buffalo, NY 14214-3079

  38. What happens now? • You will receive a confirmation email after you submit your application. • If you do NOT receive a confirmation email, you MUST follow up with the department within one week of your application submission. • Contact Pedro Sotelo-Peryea: pjsotelo@buffalo.edu • If you did NOT receive a confirmation email and do NOT follow up, your application may be considered LATE. • READ this confirmation email to be sure that you have SUBMITTED your application.

  39. Acceptance • Applications will be evaluated by the ES Promotion to Professional Sequence Committee • Decision letters will be sent out by early March • If you receive an acceptance letter, you need to respond by accepting or declining your spot in the program by stated deadline. • This needs to be in writing and returned to 211 Kimball Tower. • Acceptance is conditional based on successful completion of prerequisite courses spring semester. Grades will be checked at the end of the semester. • If you are taking any courses this spring outside of UB, you must send official transcripts to both: • Dept of Exercise & Nutrition Sciences, 211 Kimball Tower, UB, Buffalo, NY 14214-3079 • Office of Admissions, 12 Capen Hall, Buffalo, NY 14260-1660

  40. Alternate List • Students who are not initially accepted into the program may be added to the alternate list. • Students on this list will receive a letter indicating their position/number on the wait list. • We will contact you and offer you a spot if an opening should become available. This may be as late as the first day of classes fall semester. • There are no guarantees that seats will be open and we cannot predict your chances of gaining a seat. Therefore, if you are on the wait list, you should still plan accordingly for alternate plans.

  41. Spring Orientation All accepted students must attend the ES Program Orientation in early April. Program begins Tuesday, May 27, 2014 with Human Gross Anatomy (this is the day after Memorial Day)

  42.   43. Important Reminders If you do NOT receive a confirmation email, your application may not have been submitted correctly  you MUST follow up with the department within one week of submission. If your contact information changes anytime between January & March  you MUST let us know!
