Process Map for Current Dog Licensing Current map as of August 26, 2012 Dog license applications are mailed annually to existing registered dog owners by Carol Dog owner downloads and completes dog license application from health dept web site Dog owner applies for license by coming into Town Hall Dog owner sends back(via mail. Email or fax), or comes to Town Hall with required information . Dog owner completes application on site, if necessary Health Dept. staff review and process information Application is approved and info Is logging in to Town Hall’s Dog licensing computer database Application is not approved ie: Need for updated rabies vaccination; spay/ neuter verification; incorrect fee Dog license and receipt is mailed to dog owner by health dept. staff Dog license and receipt is provided to dog owner at Town Hall counter by health dept. staff Dog owner is contacted by Carol either by phone or mail that further paperwork is required before license can be issued Dog owner submits required information into Town Hall via walk-in, email; fax or mail
