  2. Background Transition Cadre • Established in 2007 with the initial goal of making 100% of the transition IEP’s within the Grant Wood AEA service area 100% compliant, 100% of the time. Interagency Stakeholders Group • Created on May 2012 – Agencies & Service Providers • United Way, Discovery Living, Goodwill of the Heartland, etc.

Reasons for the ‘IN’ • Frustrated direct service providers • Families not accessing services that were a ‘good fit’ • Unaware of services and funding • LEAs unaware of agencies/service providers • Simplification • Needed a tool to help find agencies or service providers that could meet the needs of each individual student

History • Outgrowth of the parent special focus group • Gather all services under one roof • Address the three prongs of transition - Living - Learning - Working • Free flowing atmosphere and schedule so that participants can come to all or some of the evening activities • Provide all partners in the transition process to be together and share information
