This service is for anyone wanting to break the social barriers and build social interactions with people within their community.   2. Post Hospital Care Life after a hospital discharge can be challenging to adjust to, and for clients unsure about care. DH Holistic Care provides hassle-free care from hospital transfer to home to providing 24hr support. We work collaboratively with you and your care teams, such as family or GP, to ensure you are well supported with the right care for your needs. We focus our care on your recovery and lifestyle according to your goals, preferences, and needs. • Post Hospital Care services: • Meal preparation and nutrition • Light housekeeping & maintenance • Personal care • Nursing care • Mobility around the house and community • Transportation home from hospital • Organising home modifications such as grab rails • Specialised care for people with acquired brain injuries

  3. Respite Care We have flexible respite arrangements tailored to meet both the caregiver’s needs and the person’s needs being cared for. To help support clients and their families, our respite care does not have a minimum duration. We canaccommodateyou within one of our respite homes or respite in your own home anytime and anywhere, whether you need assistance for a few hours or a couple of weeks. Our professional and caring staff are ready to provide 24 hour/7hrs respite care, or drop-in basis care, you choose, and we provide services. n addition to our 24/7 availability, our staff can assist with your daily needs during your stays with us, such as community participation, nursing services, meals, personal care, medications, and transport to and from appointments/hospital.
