  5. 1.0 Welcome • First, You Are a Customer • EMID & HIPPA Document Completion • Goto www.lifetrackllc.com • Under the spinning globe is a little KEY and a LOGIN • Login with your distributor number and password • Under the Customer Tab is • EMID (Emergency Medical ID) – This is where you imput the information on the outside of yourLifeTrack Complete Card • HIPPA (Health Information PrivacyProtection Act) Release Form

  6. 1.0 Welcome • First, You Are a Customer • Downloading the LifeTrack Welcome Package • Mouseover the EMID Link and to the right will appear the LifeTrack Welcome Package Link • Download that package • Contains the Authorization to Release Medical Records Document (HIPPA Release) • Fill out the HIPPA Release • Check all the boxes • Sign • Mail to Lifetrack LLC
