It was a frustrating task that left me with more questions than answers. Hence, the colorful slides…

    2. Questionnaires Used with Risk Assessment Questionnaire Analysis Method Information System Evaluate Questionnaires (wasn’t able to find/configure report) The documentation indicates that questionnaires can be used in Risk Assessment via what is called the “Questionnaire Analysis Method”. They can also be evaluated via Information Systems, but I was not able to find a report. The documentation indicates that questionnaires can be used in Risk Assessment via what is called the “Questionnaire Analysis Method”. 3. Risk Assessments Accessed via Exposure Profile for Work area Tied to Agents (Hydrofluoric acid (HF)) Assigned to Work areas (6-325) So what is Risk Assessments and how do Questionnaires tie into the analysis? Risk Assessment is accessed from the Work area Exposure Profile. This ties the assessment to agents which are associated with a particular work area.So what is Risk Assessments and how do Questionnaires tie into the analysis? 4. Risk Evaluation Risk Analysis Evaluation of Questionnaires IMG Assign Analysis Methods to Agent Types In order to access “Risk Analysis - Evaluation of Questionnaires”, the analysis method, Questionnaire, must be assigned to the agent types of interest. In order to access “Risk Analysis - Evaluation of Questionnaires”, the analysis method, Questionnaire, must be assigned to the agent types of interest.

    5. Risk Assessment via Analysis Once configured, you can access Risk Analysis - Evaluation of Questionnaires via the Analyze button on the Exposure Profile screen. If you are lucky and the stars are appropriately lined up you can successfully display the answers to the questionnaire in the bottom half of the screen & mark the answers as relevant for rating. Note the questions are weighted which I did when I created the “general” questionnaire. But I was unable to automate the questionnaire rating based on the answers to the specific questionnaire. Somehow I’m not quite groking how this is suppose to work! Also I’m very unclear how this works or doesn’t work when there are multiple hazards in a work area. I was not able to successfully get it to work but it was inconsistent in the ways it failed. I got the impression this functionality was not quite ready for prime time!Once configured, you can access Risk Analysis - Evaluation of Questionnaires via the Analyze button on the Exposure Profile screen. 6. Automating Analysis After I manually rated the questionnaire B “high risk”, the “Calculate Main Rating” worked. The assessment was rated as B “Hazard level 2”. Finally the “Assessment matchup” on the Exposure Profile screen rated the exposure. But notice it is only working for one hazard in this work area. After I manually rated the questionnaire B “high risk”, the “Calculate Main Rating” worked. 7. Scanning Responses into SAP One of the specific notes in the documentation that Hal asked me to investigate was the ability to scan questionnaire responses into SAP. I wasn’t able to discover much other than this menu item from the “Edit Questionnaire” screen. I suspect that we won’t find this feature very useful. But we will find the ability to associate “no” to all unanswered questions very useful. We can then mark those few questions with “findings” or “findings with correction”.One of the specific notes in the documentation that Hal asked me to investigate was the ability to scan questionnaire responses into SAP. 8. Document Management System Can be assigned to Work areas Can not be evaluated in Info System Can not be analyzed for Risk Assessment Good for emergency response plans Good for capturing questionnaire w/notes I did not put a lot of effort into exploring the Document Management System. We are using documents in other modules. There does seem to be more functionality associated with documents in more recent versions of SAP, but I didn’t investigate. I did find that documents can be assigned to work areas. But they can’t be evaluated via reports or analyzed for risk assessment. 9. Goals from User Requirements Centrally stored inspection records Both questionnaires & documents Tracking preventive & corrective actions Questionnaires via Risk Assessment Trend analysis capabilities Questionnaires via Info System Track versions Both questionnaires & documents Capture inspection findings requiring further action Questionnaires via Risk Assessment & Safety Measures Prioritized findings Questionnaires via ratings Institute-wide metric reports Neither Are the user requirements met?Are the user requirements met?
