Biophysical Profile = BPP • Ordered typically for • Decreased fetal movement • Toxemia • Diabetes • Heart decelerations • Abnormal stress test • 30 minute timed exam • Score is out of 8 points • 2 points each for the following • Breathing • Fetal muscle tone • This does not include fetal eye movement • Gross body movement • AF volume • No points, but need to check • Cardiac motion/HB • Placenta grade • Blood flow evaluation • Cervix if possible

Fetal breathing • Score 2 • Breathing for 2- 30 second episodes or • 1- 60 second episode • Watch diaphragm • Score 0 • No breathing • Most commonly not seen out of entire BPP exam • Using color for nostrils • Use M-Mode through stomach to show diaphragm motion or live clip • Factors the influence fetal breathing • Fluid exchange by fetal lungs prevents hypoplasia • Fetal breathing increases CNS stimulants and acute glucose load • Fetal breathing decreases during labor • also hypoxia, nicotine, alcohol

  4. Fetal Movement • Score 2 • 3 gross body movements • Arms, legs, kick or punch • Score 0 • No movement or insufficient for time allowed • Ask mother when was the last time they ate • If its been a couple of hours, feed mom juice or some type of sugar, if baby is still motionless not good.

  5. Fetal Tone • Score 2 • 1 episode of limb extension and flexion • Mouth opening and closing • Fingers moving, etc • Score 1 • None of the above

  6. AF Volume • Score 2 • 1 or more pockets >1cm in AP • Score 0 • No fluid • This makes it very hard for baby to move • And breathe

  7. Placental Grading • Depends on which Biophysical system is used • Score 2 • Placenta Grade 0-2 • Score 0 • Placenta Grade 3

  8. Non Stress Test • Aka NST or fetal activity test (FAT) • Fetal monitor of heart Rate during fetal movement • Score 2 • Reactive • 15 sec period of increase heart rate in 20-30 minute observation • Mom presses a button attached to the machine indicating fetal motion • 3 or more fetal movements • Score 0 • Non reactive • When this is included with the BPP Score there is a possible of 10 Points

  9. Version • Turning of breech baby to vertex • Drugs given to relax patient

  10. History of Trauma • Pain • Bleeding • Reassurance s/p accident

  11. Maternal Health Concerns • Diabetes • Hypertension • Drug abuse • Age • Genetic history

  12. Professionally • Adjust to changing patient needs and concerns throughout all stages of pregnancy
