What is Carolina Advanced Health (CAH)? • New off-campus primary care practice (the Model Practice) opened in December 2011 • Focus on improved health outcomes for chronically-ill patients • Team-based model with on-site primary care, pharmacist and behavioral health providers • Located on Hwy 54 near I-40 • Partnership between BCBSNC and UNCHCS • Aligned incentives - share costs and downstream medical expense savings • TPN practice with unique branding • Staff are TPN employees • Logo contains BCBSNC and UNCHCS branding

Major Objectives of Collaboration • Provider • More time to fully engage patients in their care • Improved access to information and tools to provide quality care • Payer • Demonstrating leadership in changing the healthcare system • Aligning incentives • Creating a sustainable, replicable model

Scope of Initiative • Select BCBSNC members only • 1500 State Health Plan members are eligible to attend • 5,000 patient panel • Enriched with chronically-ill adult population (e.g. Diabetes, CAD, HTN, Diabetes, Obstructive Lung Disease, Depression, Hyperlipidemia, and Asthma) • Targeted mailings with copay voucher to attract members • Three-year pilot beginning on December 1, 2011

Considerations for Other UNCHCS Practices • If model is successful, may positively influence the future of primary care reimbursement and delivery models in NC • New technologies will be rolled out to other practices • BCBSNC members that are not eligible or lose eligibility for CAH will be referred to a UNCHCS/TPN primary care practices • Coordination of care is a critical component for success of CAH • CAH to use WebCIS to enable common medical record with specialists
