Waiver Background • Projects originally submitted in 2012 • Providers implemented transformational projects with a focus on providing better care, in an effective manner, to save costs • Projects submitted in 2013 • More of an emphasis on outlining sustainability plan • Mid-point assessment • Providers may need to make changes

  4. Looking to the Future • What could happen in the future? • Extension • Renewal • Nothing • What would happen to your project tomorrow if the Waiver were to go away?

  5. Defining Sustainability • Sustainability – Providers ability to incorporate projects that have been implemented through the 1115 Waiver with the existing organizational structure. • How do you define sustainability?

Three Step Process • Step 1: Clarify Your Vision and Budget Costs • Write out your vision and goals for this project. • Identify costs related to keeping the project alive past DY5. • Step 2: Build Collaboration • Identify stakeholders who benefit from the project. • Keep partners informed about your progress on sustainability efforts. • Step 3: Develop Action Steps • Identify potential funding sources for sustaining your project.
