Diabetic Foot Referrals Two vascular wards with a total of 44 beds + outliers GP, community or tertiary referral • Follow-up at local hospital with: • Acute foot team • Vascular surgeon • Microbiologist Radiology MDM followed by in & out patient ward round every Monday (up to 30 patient reviews) • Follow-up at local hospital with: • Acute foot team • Follow-up at GSTT out-patient MDT clinic: • Out-patient antibiotic therapy reviews

  3. 20 week review • Number of contacts: 119 complex cases • Age range: 40 - 93 • Gender: Male 71% Female 29% • Length of stay: 4 days to 109 days • Deaths: 7
