  3. STEP 1: State Application Mail in your completed application and $92fee 60 days prior to your expected graduation. State Application: • Download online: www.doh.wa.gov • Fee: • Mail personal check, money order, or cashier’s check • Credit cards are now excepted at the front counter in Tumwater only.

  4. Application Process • Revenue • Customer service center • Program for intake • Criminal background check • Program for review • Program approval This will take approximately 4-6 weeks.

  5. Finger Print Cards If you have an out of state address on your application … Washington State now requires FBI finger print background checks.

  6. STEP 2: Register for Exam Register to take the NCLEX exam and submit the $200 fee to PearsonVue. Who is PearsonVue? PearsonVue is the company that has been contracted to provide test development and test administration services for the NCLEX Examination.

  7. What is the Candidate Bulletin? • The Candidate Bulletin contains information for all registrations and scheduling of examination appointments. • Download paper version at: www.pearsonvue.com/nclex “How do I register?”

STEP 3: Receive ATT • After you have registered with PearsonVue and the Washington State Nursing Commission has received the certificate of completionortranscriptsfrom your School of Nursing, we will make you eligible to test. • PearsonVue will then supply you with your Authorization to Test (ATT).

  10. Testing Availability The test dates vary depending on the time of year that you are testing. First time test takers must be offered an appointment within 30 days of the call. Repeat candidates must be offered an appointment within 45 days. You may decline the appointment offered and schedule later than the 30 or 45 days.

  11. Test Sites Four sites in Washington: • Seattle - 23 seats • Renton - 15 seats • Yakima - 8 seats • Spokane - 10 seats Each student has three choices of times, dates, and locations. Please make your choices as soon as possible to meet your internship and review course deadlines.

  12. STEP 4: After the Exam • Within 14 days after we received that last document that we need we will license you. Then you can go onto our web site to see if you have been licensed (www.doh.wa.gov, under the Provider Credential Search). • Wait for your license in the mail. It will take 7-10 business days after you are licensed. NOTE: Your license cannot be issued without the degree earned listed on the official transcript.

  13. Test results DO Not call DOH to see if you have passed the exam. To find out you can go onto Pearsonvue’s website and pay for your unofficial results.

  14. Paperless Licensing • Renew your license every year on your birthday. • Effective April 1, 2009 we NO longer mail out a new license when you renew. • Employers can view the updates on our website at https://fortress.wa.gov/doh/providercredentialsearch.

  15. Accommodations If you need accommodations for the NCLEX you will need the following: • A letter from you stating what you need and why you need them. • A letter from the school stating what accommodations you had while in the nursing program . • A letter from the physician who diagnosed you, what the diagnosis is, and what was recommended for the diagnosis.

TIPS ON HOW TO AVOID DELAYS • When you fill out the application, make sure it is COMPLETE and the correct fee is sent with it. • Check with the school to make sure you have signed all necessary paperwork to have your COC and transcripts sent to DOH. • When filling out the application for DOH, make sure you indicate your CURRENTnursing program and graduation date under the professional education section.
