Overview • Shift of lead responsibility from Department for Education to Department of Health and Social Care • Larger footprint of the CDOPS with a minimum 60 caseload with enabling legislation to allow two or more areas to deliver CDR functions together • Development of a new “key worker” role to act as a single point of contact with the bereaved for information on the child death review process, and who can signpost them to sources of support.

CDOP Purpose (Final Stage of the CDR Process) • To identify any matters relating to the death, or deaths, that are relevant to the welfare of children in the area or to public health and safety • To consider whether action should be taken in relation to any matters identified. • To prepare and publish reports on: • what has been done as a result of the child death review • how effective the arrangements have been in practice;

CDOP role and membership • A multi-agency panel set up by Child Death Review Partners • review the deaths of all children normally resident in that area • Death of any live-born baby where a death certificate has been issued • Attended by senior professionals across agencies who have had no involvement in the case • Local Authorities and Clinical Commissioning Groups to make arrangements for CDOP

Child Death Review Meeting (CDRM) • multi-professional meeting where all matters relating to an individual child’s death are discussed by the professionals directly involved in the care of that child and any professionals involved in the investigation into his or her death. • Should be “ flexible and proportionate” • already occurs in different formats e.g.: mortality and morbidity meeting, perinatal mortality meeting, local case discussion, the final case discussion (SUDI). • In different settings the meeting might discuss one child or several children. • outcome of the child death review meeting (draft Analysis Form (C)) should be forwarded to the relevant CDOP, to provide a “standardised output “

Child Death Review Meeting (CDRM) Purpose: • to review the background history, treatment, and outcomes of investigations, to determine, as far as is possible, the likely cause of death; • to ascertain contributory and modifiable factors across domains specific to the child, the social and physical environment, and service delivery; • to describe any learning arising from the death and, where appropriate, to identify any actions that should be taken by any of the organisations involved to improve the safety or welfare of children or the child death review process; • to review the support provided to the family

Revision of additional requirements to address “complex” circumstances, including: • Deaths of UK-resident children overseas • deaths of children with learning disabilities (LeDeR) • Deaths from suicide • Deaths of children in adult healthcare settings • Deaths in inpatient mental health settings • Deaths in custody

Key roles • Designated Doctor for Children’s Deaths a senior paediatrician has responsibility for: • the child death review process and advising the CDOP in relation to themed panels • co-ordinating responses and health input to the child death review process, across a specified locality or region. • CDOP should be chaired by someone independent of the key service providers in the area. • Key worker - acts as a single point of contact for the bereaved family • for information on the child death review process • can signpost them to sources of support • What are the local implications?

CDOP Footprints • CDOP footprints should be locally agreed and aligned to existing networks of NHS care and other child services, and should take account of agency and organisational boundaries • CDOPs should typically cover a child population such that they review 60 child deaths each year. • What are the local implications?

Connections • Learning Disabilities • When notified of the death of a child or young person aged 4-17 years who has learning disabilities, or is very likely to have learning disabilities but not yet had a formal assessment for this, the locality CDOP should report that death to the LeDeR programme • Suicide and Self-harm • CDOP will report to the National Confidential Inquiry into Suicide and Homicide by People with Mental Illness in all cases of child death that they believe might be due to suicide or self-harm
