  3. Dental Plans Information • IBBP insures nearly 8,000 employees in the dental plan program. • Three dental plans to choose from. Employers with 10 or more eligible employees may offer employees up to two plans; employers with 50 or more eligible employees may offer three. • Premium increases have been on average 2% or less for the last three years. • Same eligibility as health plan (IBBP)

  4. Dental Plan I • $1500 maximum coverage/person/contract year • Included coverage for: • Teeth straightening (50% coverage, $2000 lifetime max) • Dentures and Bridges • Gum and Bone Disease • Pays 100% UCR of check-ups & teeth cleaning (2x year) • Crowns covered at 80% - most plans are not as generous. • Also covered at 80%: Routine and Restorative Services; Root Canals (Endodontic); Most Periodontal Services; High Cost Restorations; Routine Oral Surgery **All plans include a $25/person deductible, which is waived for check ups and teeth cleanings, as well as other preventive services.

  5. Dental Plan II • $1500 maximum coverage/person/contract year • This plan also is available for retirees (age 55+) • Pays 100% UCR of check-ups & teeth cleaning (2x year) • Crowns covered at 80% - most plans are not as generous. • Also covered at 80%: Routine and Restorative Services; Root Canals (Endodontic); Most Periodontal Services; High Cost Restorations; Routine Oral Surgery **All plans include a $25/person deductible, which is waived for check ups and teeth cleanings, as well as other preventive services.

Group Term Life Insurance • Available to employees working 20+ hours/week (Employer may set higher) • Available in increments of employees salary (up to 6x) to a maximum of $500,000 per employee. • Rates are based on the average age of each individual Bank’s employee base. • IBBP presently insures over 8,000 employees in our Group Term Life insurance program.

Group Term Life Insurance • Coverage is convertible or portable to either permanent or term insurance without medical evidence at termination of employment. • Beginning at age 65; individual coverage amounts begin to reduce in an effort to control increasing cost of the coverage to the entire group. • Waiver of premium and Accidental Death and Dismemberment included at no additional cost.

Accidental Death and Dismemberment • Included at no additional cost to Bank / employee. • Full amount of AD&D is paid if there is loss of life or loss of multiple appendages. • One-half amount of AD&D is paid for loss of one appendage. • Extra seat belt provision – If you are killed in an accident while wearing your seat belt, the company will double your accidental death benefit up to $50,000.

  18. Additional Group Term Life Insurance • Available in increments of the employees salary (1, 2, or 3x) to a maximum of $200,000 per employee. • Paid for by the employee. • Rates are based on the actual age of the employee. • Insurance amount based on current salary. • 1x available w/o evidence of insurability when first eligible. Late enrollees and requests for 2x and 3x require submission of satisfactory E of I.

  19. Dependent Life Insurance • The Bank has two plan choices available. • $10,000 for spouse / $5,000 per child. ($2.15/mo) • $25,000 for spouse / $10,000 per child.($4.30/mo) • The employee then has two additional plan choices that can replace the Bank’s choice: • $35,000 for spouse / $10,000 per child. ($6.15/mo) • $50,000 for spouse / $10,000 per child. ($8.05/mo) • Increasing dependent coverage requires submission of satisfactory evidence of insurability.

  20. MEDEX® Travel Assist • Provides assistance, referrals, transportation and evacuation services for travelers. • Offers pre-travel assistance • No enrollment necessary. All employees with an IBIS GTL plan (including spouse and children under 19 or through age 24 if a registered full time student) are automatically enrolled. • Most services available if you are 100 or more miles from home. • All services provided by MEDEX Assistance Corporation. • *This is not an insurance plan.

  21. Short Term Disability • Benefits become payable when employee becomes disabled as a result of a non-work related accident/illness. • Benefit amount is equal to 60% of weekly salary to a maximum of $1,000 benefit per week. • Coverage provided 24 hrs per day / 365 days per year. • Benefits will be subject to FICA and withholding taxes if provided 100% by the employer.

Short Term Disability • Four plan choices differentiated by the elimination period and duration of benefit. • 1st day accident / 8th day illness / 13 weeks • 31st day accident / 31st day illness / 13 weeks • 1st day accident / 8th day illness / 26 weeks • 31st day accident / 31st day illness / 26 weeks • Also, there are different maximums for coverage: $500/week or $1000/week

  23. Long Term Disability • Benefit amount is equal to 66 2/3% of monthly gross salary with a maximum benefit of $8,000/mo. • Benefits begin on either the 91st (LTD90) or 181st day (LTD180) for accident and sickness. • Benefits are integrated with government programs such as Social Security and Worker’s Comp.

  24. Long Term Disability • You are considered disabled if, as a result of sickness, injury, or pregnancy, you are unable to perform with reasonable continuity the material duties of your own occupation during your benefit period. • Benefits will continue to be paid to age 65, or your Social Security Normal Retirement Age (SSNRA), or longer if a full-time employee.

  26. Supplemental Benefits Insurance provided by American Heritage Life Insurance Company, a member of Allstate Benefits Available for any individuals age 18 – 65 Coverage can be written on: • Employee only • Spouse only • Family unit Coverage is portable. Available on a payroll deductible basis.

  27. Cancer and Specified Disease Insurance Helps financially protect those with a family history of cancer. Benefit payments go directly to the insured. Helps minimize the non-reimbursed expenses such as travel, lost wages, special diets, in-home care, deductibles, co-insurance, co-payments, etc. Provides benefits from diagnosis through treatment. • Hospitalization related • Transportation and lodging • Extended care • Wellness benefit • “Basic” plan cost - $18.96/mo.

  28. Accident Insurance Protects against the financial strain of work loss due to an accident. Helps cover bills during work leave due to an accident including, car payments, card payments, mortgage/rent payments, and other recurring expenses. • Covers the “full cycle” of the accident. • 24 hour coverage • Built in accidental disability benefit Covers everything from an accidental death to a broken finger. Cost for individual coverage - $26.12/mo

Critical Illness Insurance Three different “buckets” of benefits available • Benefits for Heart related illness • Benefits for major disease related illnesses • Benefits for cancer initial diagnosis Coverage purchased in lump sum amounts from $5,000 - $50,000 Wellness benefit included Coverage available for individual, single parent family, or full family.

Heart / Stroke Insurance Pays benefits for the necessary care and treatment of a heart attack or stroke, and other conditions directly caused or aggravated by heart disease Coverage begins prior to diagnosis and continues through treatment. Coverage for: • Hospitalization • Surgery • Second opinions • Transportation and Lodging • Cost of “Basic” plan - $17.96/mo

  31. Life Insurance Universal Life Insurance • Available in increments of $10,000 from $10,000 to $150,000. • Can build cash value with current interest rates. • Living benefit rider included Term Life Insurance • 20 year term period • Insurance amounts beginning at $10,000. • Does not generate cash value • Living benefit rider included.
