Course Selection Form • There are three parts to the front of the form: • Name and Contact Information • Graduation Checklist – you require a minimum of 20 courses to graduate from high school • Course Requests including Alternate Elective Requests and which course, if any, you are planning to take in summer school • On the back of the form is a list of the courses and their codes. • You are not allowed to repeat a course at Dr. Charles Best

  29. Study Block (Grade 12s) • Obtain a “Study Block” Request form from your counsellor • Must have at least 92 credits towards graduation • Must have an academically demanding schedule ( 3 academics each semester) • Must be signed by your parent and counsellor • Must be approved by an administrator • Must be returned with your course selection sheet before February 22nd
