  3. Important Benefit Reminders Annual Open Enrollment (Excludes retirement) Scheduled from Monday, October 14, 2019, 12:01am EST through Friday, November 1, 2019, 6:00pm EST. Coverage changes take effect January 1, 2020 Annual Benefits and Wellness Fair, in Gainesville (Monday, October 28, 2019, 9:00am–3:00pm, in the Champions Club, at Ben Hill Griffin Stadium) Other Benefits and Wellness Fair locations in Florida –https://www.mybenefits.myflorida.com/health/open_enrollment_for_the_2020_plan_year/2019_open_enrollment_benefit_fair_schedule

Prescription Drug Plan adopts Formulary Management For participants of the State Employees’ Prescription Drug Plan (RX Plan), formulary management will be implemented and effective January 1, 2020. CVS Caremark will provide notification to impacted participants which will include a list of alternative preferred drugs covered under the RX Plan, by December. The Preferred Drug List, Maintenance Drug List, Specialty Drug List, and a list of drugs subject to prior authorization approval for coverage are located on the State of Florida specific CVS Caremark website and the log-in site (see the Quick Links box for the web address). Important Reminders Healthcare FSA and Limited Purpose FSA • 2019 Plan Year FSA funds must be used by 12/31/19 • Remaining FSA balance after 12/31/19 will carry over up to $500 into the next plan year • Funds over the $500 carryover amount will be forfeited • Annual FSA Max can still be selected even with a carryover • $500 carryover will be added to your annual election • Deadline for claims submission stays the same – by 4/15 each year • Total balance (annual election + carryover) will only show on Chard Snyder portal, and not People First • Canceled FSA, but have a carryover at the end of the year – the FSA will carryover year to year until funds are depleted. The Benny Card remains active and the account can be reviewed on the Chard Snyder portal

Important Reminders Other FSA News… • Dependent Care FSA does NOT have a $500 carryover like the Healthcare and Limited Purpose FSA • Grace period still provided for Dependent FSA’s • Claims for FSA accounts (healthcare, limited purpose and dependent care) must be filed by 4/15 each year • Chard Snyder offers a Live Chat Feature • Available in the FSA & HSA sections on the People First portal • Chat with a Chard Snyder rep. about anything you might call or e-mail about • Representatives available for chat Monday –Friday, 8:00am-5:00pm EST

UFSelect Plans Eagles Direct Reimbursement Dental Plan--Slight increase in monthly rates for 2020 for (spouse, child(ren), and family) No change for employee rate Guaranteed Issue Life during OE for the following: • UFSelect Life Insurance- Guaranteed Issue for Spouse Life in increments of $5,000.00 to $50,000.00 no Evidence of Insurability (EOI) required • Guaranteed issue for Child Life In increments of $5,000.00 up to $25,000.00 with no Evidence of Insurability (EOI) *Note: If previously denied—must complete EOI for any amount requested. Active work provision for add/increase in coverage at OE.

State Plans Health Savings Account (HSA) • Contribution maximum increased for 2020 • $3,550 individual / $7,100 for family • Includes state’s contribution Dental Plans • MetLife and Ameritas dental plan rates will increase for the 2020 plan year. Check your annual benefits statement for these changes. . Humana Select 15 will be known as Humana HD205 and Sun Life Freedom Advance will be known as Sun Life Indemnity PPO Benefit Scout-Option Life Selection Tool Securian Financial is offering a new tool to help you compare your benefit options, get cost estimates, and determine what life insurance may be a good fit for you! Securian Financial will mail out more information about Benefit Scout™ closer to Open Enrollment. Be on the lookout!

State Plans Enhancements for state health plan participants: Shared Savings Program--cost saving innovations and options • Healthcare Bluebook and SurgeryPlus • Automatic enrollment in the program • Rewards credited to HRA, FSA, or HSA (health savings account) • Program details sent directly to participants from DSGI/PeopleFirst Chard Snyder Benny cards--new design for cards • Continue to use your old card until it expires • New cards sent automatically
