  3. Introduction • Every minute of every day somewhere in the world a woman dies while pregnant or while giving birth. • About 99% of maternal deaths occur in countries with high levels of poverty, and Africa accounts for over 50% of these deaths 12.

  4. What is most unfortunate is the knowledge that most maternal deaths are preventable. What’s lacking?

  5. A bicycle ambulance, Malawi-World Vision In order to prevent maternal mortality on a large scale, translational research is needed to bridge the gap between medical advances and community-based interventions that are feasible in low-income nations.

Background Maternal death is defined as “the death of a woman while pregnant or within 42 days of termination of pregnancy, irrespective of the duration and site of the pregnancy, from any cause related to or aggravated by the pregnancy or its management but not from accidental or incidental causes” 13.

  7. Millennium Development Goal (MDG-5) Despite the global commitment to reduce the maternal mortality ratio(MMR) by 5.5% each year (MDG-5), the current rate of decline is less than 0.4% per annum14.

  8. Maternal mortality: direct causes Almost 75% of maternal deaths are caused by direct complications as follows: • Hemorrhage (25%) • Infection (15%), • Unsafe abortion (13%) • Eclampsia (12%) • Obstructed labor (8%) -(WHO, 2008).

  9. Maternal mortality: Underlying causes In an analysis using data provided by the WHO for 168 nations15, strong positive correlations were found between MMR and: • infant mortality rate • total fertility rate • poverty rate and • adolescent fertility rate .

  10. Strong negative correlations were found between MMR and: • percentage of births attended by a skilled attendant • percentage of women using contraceptives • adult literacy rate.

  11. Eighty-one percent of the variation in MMR was explained by differences in: • Infant mortality rate • Percent of births attended by a skilled provider • Percent of women using contraceptives • Total fertility rate • Adolescent fertility rate • Adult literacy rate • Poverty

  12. Addressing both direct and underlying causes is essential to reducing maternal mortality.

  13. Research Objective This systematic review of the literature identifies the medical and community-level interventions that have been established as effective in reducing maternal mortality.

  14. Methods A search conducted using “maternal mortality” as key words and limited to the publication years 1998-2008 resulted in 6975 titles from CINAHL and Medline databases See handout: Figure 1

  15. A total of 16 studies which were included in the analysis met the inclusion criteria of being intervention studies (experimental or quasi-experimental) aimed at reducing the risk of maternal death.

  16. Results • Nine studies resulted in a decreased risk of maternal deaths (see handout Table 1). • Six studies did not significantly alter maternal mortality outcomes (see handout Table 2). • The results of one study were associated with an increased risk in maternal death (see handout Table 3)

Discussion This systematic review identified 4 medical strategies which were shown to significantly reduce the risk of maternal death: • Magnesium sulphate and H.E.L.P. apheresis for pre-eclampsia and eclampsia16, 17, 18 • Oral misoprostol for postpartum hemorrhage22 • Vitamin A supplementation o reduce risk of maternal morbidity23

The WHO should classify eclampsia and postpartum hemorrhage as priority conditions given that they are the most prevalent cause of maternal death. • In low-income nations where the majority of births are at home, community-based interventions with strong evidence of effectiveness should be replicated and tailored to specific communities.

  20. Study Limitations • Since the methodology required strict inclusion and exclusion criteria, many research studies were excluded which would have contributed to our understanding of the factors influencing maternal mortality. • This review focused on direct causes of maternal mortality, but we should not overlook the significant impact (on maternal mortality) of indirect causes.
