Shealsooffersassessmentofautisticspectrum disorder(ASD)andmanagementof associatedanxiety,depressionandchallenging behaviour.

PSYCHIATRIC DISORDERS Aninitialassessmenttypicallytakesbetweenonetooneanda halfhoursbutcanvary,dependingonindividualcircumstances.It includesconversationwithparents,usuallyinthepresenceofthe youngpersoninordertokeepthingstransparent.Ialsotalkwith theyoungperson,eitherwithorwithoutparentspresent, dependingontheyoungperson’sageandpreference.Ithen discusswithparentsandtheyoungpersonmyrecommendations forthebestwayforward,includingpsychologicalsupportand/or medication.IftraitsassociatedwithADHDorASDareidentified,a follow-onassessmentfortheseconditionsisrecommended.A prescriptionisprovidedattheappointmentiftheyoungpersonis commencedonmedication,unlessthereareunderlyinghealth conditionsthatrequirefurtherexamination/investigations,or baselineinvestigationsarerequiredbeforeprescribingthe medication.Areportontheassessmentfindingsisprovided followingtheinitial appointment.

ATTENTIONDEFICITHYPERACTIVE DISORDER Priortoassessment,somewrittennarrativeinformationfrom parentsandfromtheyoungperson’sschool/collegeis requestedviasuppliedsemi-structuredforms.Inaddition,in maybenecessaryforparents,school/collegestaffand,where oldenough,theyoungpersontocompletesupplied screeningquestionnaires.Theassessmentappointment typicallytakesbetweenoneandahalftotwohours,though canvary,dependingonindividualcircumstances.Thesession involvesconversationwithparentsandtheyoungperson aboutcurrentdifficulties,aswellasbackgroundinformation, includingearlydevelopment.Ifadiagnosisisreached, treatmentoptionsarediscussed,includingbehaviouraland environmentalstrategiesforparentsandteachers,and medication.Familiesareprovidedwithvarioussourcesof furtherinformation,includingliteratureonthedisorderand usefulwebsites.Shouldmedicationberecommended, abrief physicalexaminationisconductedandaprescriptionis usuallyissuedonthesameday,unlessthereareunderlying healthconditionsthatrequirefurther examination/investigations.Areportontheassessment findingsisprovidedfollowingtheappointment.

AUTISTICSPECTRUMDISORDER Priortoassessment,somewrittennarrativeinformationfromparentsandtheyoungperson’sschool/collegeisrequestedviasuppliedsemi-structuredforms.Itmayalsobenecessaryforparents,school/collegestaffand,whereoldenough,theyoungpersontocompletesuppliedscreeningquestionnaires.Theassessmentprocessrequiresthree appointments.Adevelopmentalhistorysessionforparentstodiscusswithmetheyoungperson’searlydevelopmentandcurrentbehaviours,whichcantakebetweenonetooneand a halfhours.IfIhavenotmettheyoungpersonpreviously,wealsomeetbriefly at thisappointment.Theyoungpersonattendsasessionwithoneofmycolleaguestocompleteastructuredplay-basedassessmentforautism,knownastheAutismDiagnosticObservationSchedule(ADOS).Thistakesuptooneandahalfhours.Afollow-upappointmentwithmetodiscusstheoutcome of assessment andmyrecommendations,whichcan take uptohalfanhour.Ialsoprovideinformationaboutthedisorder,aswellasotherusefulresources.PleasenotethatanadditionalinitialpsychiatricassessmentmayberequiredpriortoassessmentforASDforcasesreferredviaprivatemedicalinsurance.Thismayalsobenecessaryforself-fundingparentsifthereferralisnotsufficientlyclearabouttheyoungperson’ssymptoms,orifthereareassociatedissuessuchasanxiety,depressionorchallengingbehaviourwhichneedtobe addressedbeforeanassessmentforASDcanbeconsidered.

COMBINEDASSESSMENTAND TREATMENTFORASDANDADHD ThisissimilartoanassessmentforASD,exceptthat thedevelopmentalhistoryappointmentmay sometimestakeuptotwohours.Ifadiagnosisis reached,wediscusstreatmentoptions,including behaviouralandenvironmentalstrategiesforparents andteachers,andmedication.Writteninformation aboutthedisordersandotherusefulresourcesare provided.Shouldmedicationberecommended,a briefphysicalexaminationisconductedatthefollow- upappointment.Aprescriptionistypicallyissuedon thesamedayunlessthereareunderlyinghealth conditionsthatrequirefurther examination/investigations.Areportonthe assessmentfindingsisprovidedafterthefollow-up appointment.AswithanASDassessment,aninitial psychiatricassessmentmightberequiredinadvance forparticularcases.

  8. REVIEWAPPOINTMENTS Thepurposeofthesesessionsistoreviewmentalstate and/ormedicationaspartofongoingtreatmentforany oftheaforementionedconditions.Theyusuallytakea maximumofhalfanhour.
