2. Cardiac Care Pathways = Better Outcomes • Nextron Infusion Services provides a customized cardiac clinical pathway for each patient transitioning home on inotrope therapy, from day 1. Our cardiac care pathways program focuses on a safe, multi-disciplinary approach to transitioning the advanced heart failure patient from any in-patient setting to their home.

3. Cardiac Care Pathways focus on Inotrope Therapy • The goal of inotropic agents is to enhance cardiac output by reducing preload and afterload. Patients who cannot be weaned from intravenous to oral therapy may require continuous infusion of inotropic agent such as dobutamine and milrinone. • They are positive inotropic agents used for the management of patients with stage III and IV congestive heart failure.

4. Pathways are customized to each individual patient…. • Our goal is to identify all of the patient care needs upon initial assessment and customize each of our cardiac care pathways by a trained cardiac care nurse.

5. Every aspect is assessed for a seamless transition…. • This includes a thorough physical, psychosocial, and environmental assessment from our dedicated cardiac care RN. Each patients clinical status is monitored daily throughout this transitional process home.

How does it remote monitoring or telehealth work? • A physician prescribes telehealth along with inotrope therapy indicated. • Nextron installs the telehealth equipment and trains the patient on its proper use. • The patient takes their vital signs and answers personalized, clinician-directed surveys daily. • Results are automatically sent to a website via a landline or cellular signal. • Nextron’stelemonitoring nurse monitors the patient daily and alerts the patient and care team when results fall outside pre-defined limits. • Physicians receive trending reports from Nextron, or they can access patient information through a secure web portal.
