The 5 Dysfunctions of a team This occurs when team members put their individual needs (such as ego, career development, or recognition)or even the needs of their department above the collective goals of the team Because of lack of real commitment and buy in team members develop an avoidance of accountability. Without commitmetnto a clear set of actions team members find it difficult to call each other to account for the good of the team. A lack of healthy conflict is a problem because it leads to the 3rd dysfunction, without having aired their opinions in the course of passionate debate , team members rarely if ever buy in and commit to decisions even though they feign agreement in meetings. Failure to build trust is damaging because t sets the tone to the second dysfunction. Teams that lack trust are incapable of engaging in unfiltered and passionate debate of ideas. Instead they resort to veiled discussion and guarded comments. Essentially stems from an unwillingness to be vulnerable with in the group. Team members who are not genuinely open with one another abut their mistakes and weaknesses make it impossible to build a foundation for trust .

Team members quickly and genuinely apologise to each other when they say or do something inappropriate or possibly damaging to the team. __ 5. Team members genuinely make sacrifices in their area for the good of the team __ 6. Team members openly admit their weaknesses and mistakes __ 7. Team meetings are compelling and not boring __ 8. Team member's leave meetings confident that their peers are completely committed to the decisions that were agreed on, even if their was initial disagreement __ 9. Morale is significantly affected by failure to achieve team goals __ 10. During team meetings, the most important - and difficult – issues are put on the table to be resolved __ 11. Team members are deeply concerned about the prospect of letting down their peers __ 12. Team members know about each others personal lives and are comfortable discussing them __ 13. Team members end discussions with clear and specific resolutions and call to actions __ 14. Team members challenge one another about their plans and approaches __ 15. Team members are slow to seek credit for their own contributions, but quick to point out those of others.

Scoring A score of 8 – 9 Per area indicates a strength in this area A score of 6-7 indicates that some work needs to be done in this area A score of 3 – 5 indicates that this area needs to be addressed Regardless of the score these areas always need to be worked on. Because dysfunction can creep in at any time.
