  6. NFS 2 : Anémie macrocytaires Anémie macrocytaire arégénérative Alcool ? Hypothyroïdie Myélogramme LAM, MDS… Mégaloblastose Dosage B12 - Folates

NFS 2 : Anémies macrocytaires carentielles • Réserves : • Folates : 3-4 mois • B12 : 3-4 ans • Etiologies Apports Folates : malnutrition, OH B12 : végétariens stricts Absorption : Folates : jéjunum prox B12 : iléon terminal + FI sécrété par estomac Malabsorption Maladie coeliaque, whipple : Folates Crohn : B12 +/- Folates Biermer : B12 Chirurgie Besoins augmentés Grossesse, OH, Cancers Iatrogène Folates : barbituriques, dihydan, MTX, malocide, bactrim B12 : colchicine, metformine

  9. NFS 3 • Hyperlymphocytose isolée  LLC • Diagnostic simple = 1 examen (phénotypage) • Rappel : Classification de Binet = 5 aires ganglionnaires dont certaines bilatérales Tête et cou Axillaire Foie Rate Inguinale

  10. NFS 3 • Immunophénotypage Score de Matutes : LLC si 4 ou 5 points

NFS 3 • Complications de la LLC • Anémies : • AHAI • Erythroblastopénie (auto-immune) • Hypersplénisme • Envahissement médullaire • Infections • Insuffisance médullaire • Transformation en lymphome de haut grade (Richer) • Transformation en Leucémie prolymphocytaire • Acutisation exceptionnelle
