Policies / Procedures • Request and Approval Process for Standard Cognos Reports • The functional user completes report request form and submits to functional team lead • Functional team lead submits approved request form to Reporting team • Report request is placed in a queue that is prioritized weekly by the University VP • Functional team lead approves final output

Policies / Procedures • Label Process • Follows same process as report request • Reporting team completes report in an excel format and merges with word • Functional team lead validates output and sends to mailing services to be printed • Mailing service sends label to requestor • Charge back to requestor
