  5. Submission Guidelines • Student must submit all documentation via Desire2Learn. Submit the following in the 1st week documentation slot as a single document: • Student Name • Student email link: 'csu' email required, other email optional • Learning Objectives

Website Guidelines (Contd.) • Site Supervisor Name     • Host Organization • Name • Address • Website (If available) • County      • Site Supervisor • Phone number and alternate Number      • Fax Number • Email Link • Description of Company (see Section 1 of Final Report)     • The 3 objectives from Experiential Agreement

  7. Weekly Time Log Each "student/intern" is responsible for maintaining an accurate and current Internship Time Log. The Time Log will be periodically checked for student progress throughout the semester. • The Weekly time log must account for all time spent on the internship since the beginning. • The total number of hours until the end of that week must be recorded.

  8. Weekly Status Reports-Daily Journal • A detailed description of your daily activities. • Each weekly report should only detail activities for that week. • Note that the status report is a separate document from the time log. The status reports are not a substitute for the time log.

Status Reports-Daily Journal • A example detailed description of your daily activities: • June 15th, 2014 – 1:00pm – 4:00 pm • Met with supervisor to discuss available software products and implementation guidelines. Presented cost analysis for the top three vendors to include total cost, time frame limitations and technical scope requirements. Determined upcoming milestones and had all parties agree to specific dates for testing and implementation. Specific updates will be forthcoming regarding testing and installation of selected software products and applications.

Project Management; ITFN 4014 only) All Four (4) Experiential Documents

  11. “Most meaningful Learning Objective“Written Report Each "student/intern" will independently prepare a professional written report describing their “most meaningful Learning Objective" which was stated at the beginning of the semester and identified in your Experiential Learning Agreement • Cover Page • Single Space • 1,000 words (minimum) • Must be signed by Site Supervisor • Must be included in Deliverables Binder

Section ReportsSection Report Must Be Signed By You & Your Site Supervisor • Section 1 • General Description of your working environment • Section 2 • Identify the objective(s) / problem you are, will be, working on • Section 3 • Identify decision criteria which is important to resolving the problem/objective • Section 4 • Developing alternatives • Section 5 • Analyze alternatives • Section 6 • Select the best alternative • Section 7 • Implementing the alternative • Section 8 • Evaluating the decision effectiveness

  13. Final Time Log Each "student/intern" is responsible for maintaining an accurate and current Internship Time Log. The Time Log will be periodically checked for student progress throughout the semester. • Printed Hard Copy of Final Time Log (must match weekly Time Logs) • Must be signed by both the student andSite Supervisor • Must be included in Deliverables Binder

  14. Task List/Gantt Chart Each "student/intern" will construct a task list/gantt chart describing how the objective was accomplished and what tasks were performed. Many of the tasks can be tracked from the Status Reports that are maintained weekly. • Printed Hard Copy of Final Task List (ITFN 4014 students only) • Must use Project Management software for construction. You may obtain MS Project Professional software from the ELMS web site at http://msdn01.e-academy.com/clayton_infomath/ • Must be included in Deliverables Binder

  15. Minimum Hours • ITFN 2012 • Each "student/intern" is required to work a minimum of 150 hours for a 2 credit hours Cooperative. • ITFN 4014 • Each "student/intern" is required to work a minimum • 200 hours for a 3 credit hours Cooperative or • 400 hours for a 6 credit hours Cooperative • If you are registered for 3 credits and work 400 + hours you will still receive only 3 credits
