  4. EVALUATION • Scheduling • Team members • Devices • Time • Place

  5. Evaluation • Device Determination • Review of video conference • Review of observation forms • Review of parent reports • A picture or short video of student • Request of devices from MITS loan closet and vendors • Problems?

  6. REPORT • Medicaid Funding? • Private Funding? • School Funding? • What is included? • Where is the data obtained from? • Important to complete this as soon as possible after the evaluation!

  7. FUNDING • Funding Packets • Permission to request funding from insurance • Driven by companies • Require physician prescription • Require client information sheets • Denials • Addendum

IMPLEMENTATION • Technical Skills • Obtain the device and make sure the staff is aware of this (family should contact provider) • Provide opportunity for “opening the box” • Support through vendor resources • Insurance • Functional Skills • Example: DynaVox form • Build off of the success the student has using the device functionally
