  5. Qualification Designations* • State Champion (SC) • Premier League • Southern (SPL) • East (PLE) • West (PLW) • Central (PLC) • Wild Card (WC) *Located on top right of Regional Roster

  6. Team representative brings: • Player/Coach/Mgr. passes • Region III Medical release forms (original and current year) • Eligibility Verification form • Code of Conduct form • Release of Liability form • Region III Concussion form

  7. Volunteer DutiesPlayer Passes: • Verify club name, team name, age division, birth date, picture, laminated, validation by registrar/state rep.

  8. Volunteer DutiesForms: • Ensure information on roster matches pre-printed info on checklist • Verify Region III medical releases for all players and signed by parent/guardian

  9. Volunteer DutiesForms (cont’d): • Ensure Eligibility Verification form is signed by coach and manager and pull pass of any players or coaches serving suspension • Ensure that Release of Liability form is signed by each parent/guardian (or player if 18+) • Ensure the Region III Concussion form is signed by Manager and Coach.

  10. Volunteer DutiesCredentials: • Verify proper USYS pass for each credentialed coach/manager • Allow no more than four bench credentials per team • Verify each credentialed person signed Release of Liability form

  11. Volunteer DutiesWrap-Up: • Bind all players passes that have COMPLETE paperwork with red security band – alphabetically with manager last. Coach passes will NOT be bound with team • Write number of security band on checklist • Get printed name, signature, and cell number of team representative • Sign that process is complete

  12. Volunteer DutiesWrap-Up (cont’d): • Return to team representative: • Player/mgr. passes with red security band • Coach pass(es) • Medical release forms • Validated credentials • Parking passes • Goody bag pick-up slip • T-shirt preorder slip

  13. Volunteer DutiesWrap-Up (cont’d): • Keep: • Checklist • 3 rosters (League, State Cup, Regional) • Cards for suspended players/coaches • Unvalidated credentials • Eligibility verification form • Region III Concussion form • Release of Liability form
