    1. Ellenton Health Clinic Farmworker Health Services Colquitt County Board of Health Serving Brooks, Colquitt, Cook & Tift Counties

    2. History The Georgia Farmworker Health Program was established in 1990 with funding from the Bureau of Primary Health Care under Section 330 of the Public Health Service Act.

    4. Fifteen Years of Growth From one room at the Colquitt County Health Department to a small converted firehouse to todays half a million dollar structure funded by the Georgia Department of Community Affairs, Ellenton Clinic continues to grow.

    5. Farmworker Family Health Program: Summer Project Fifteen years of academic partnerships have provided health care to all children enrolled in migrant summer school in Colquitt County as well as providing episodic care to farmworkers in various camps and communities served by Ellenton Clinic.

    6. Emory Universitys Nell Hodgson School of Nursing is the lead academic entity providing coordination for the various universities and colleges participating in the Project. The School of Nursing provides family nurse practitioner and undergrad nursing students with faculty oversite. Students work under the protocols of Jacqueline H. Grant, MD, MPH, MPA.

    7. School of Nursing students, along with students from Georgia State University Physical Therapy and Psychology Departments, and Darton College and Clayton State University dental students provide approximately four hundred students with complete physicals, including dental care, sealants, and developmental testing over a two week period.

    8. Care Under the Toughest Conditions The same students work sometimes as late as midnight providing care to farmworkers and their families.

    9. Clinical hours received by these students create a valuable resource for Georgias medical community. Learning to work under adverse conditions prepares them for any medical emergency that Georgia may have.

    10. Partnering To Provide Additional Resources Ellenton Clinics various partnerships have established an After School program with Communities In Schools, a Lunch and Learn Program with University of Georgia Children, Youth, and Families At Risk grant, and a Nutrition Program funded by UGA/College of Family Consumer Sciences.

Clinic Services Provided at Ellenton Clinic Primary Care Services Episodic care including sutures, wound care, minor emergency services Most labwork can be done onsite Chronic care including management of diabetes, hypertension, and other chronic healthcare problems Two UGA Expanded Food Nutrition Program educators onsite who are able to provide health education in the homes of nutritionally challenged patients to assist them to reach prescribed health goals For clients lacking personal vehicles, transportation is provided via clinic vans

    12. Outreach: Education and Episodic Care Ellenton Clinic maintains bilingual outreach staff who visit camps and communities in all four counties who are well-versed in resources needed to keep the farmworker family healthy and strong.

    13. Reaching New Farmworkers Every Day Since May 2009, Ellenton Clinic has received two grants from the American Recovery and Reinvestment Act. The first grant provided funds to hire a Licensed Practical Nurse for the Outreach Program that will be funded until March of 2011. A second Capital Improvement Grant will allow the Clinic to purchase a smaller mobile medical unit for daily episodic care.

    14. Fees Using a sliding fee scale based on Federal Poverty Guidelines, the minimum consultation fee is $25. Any referral for services outside the clinic is $15. Medications are provided via on-site pharmacy at a cost of $5 for each prescription. The two farmworker clinics run by Southwest Georgia Health District 8-2 (Ellenton Clinic and Decatur Farmworker Health Program) are the only two clinics able to purchase drugs under a state contract, which allows for substantial savings for the program and the farm workers.

    15. Qualification In order to qualify for farmworker health program services, fifty-one percent (51%) or more of a familys annual household income must come from work in agriculture. Patients are required to provide proof of employment via checkstub or a letter from their employer.

    16. All of Georgia Benefits! The farm worker health program benefits Georgias agriculture by ensuring that there are safe and healthy workers. It also benefits our medical community by providing episodic and chronic care for people who are traditionally uninsured, thereby lowering costs related to emergency room visits and lessening the burden on society.
