CASO CLINICO • MASCULINO • 25 AÑOS DE EDAD • PILOTO • TRASPLANTADO RENAL 6 MESES • PREDNISONA 5 mg y TACROLIMUS 2 mg/d • 6 MESES PREVIOS HABIA RECIBIDO ITRACONAZOL POR 2 LESIONES FACIALES Y ENFERMEDAD PUL MONAR. • EVOLUCION DE LA LESION INTRANASAL 6 SEMANAS ASINTOMATICA • DERMATOSIS LOCALIZADA INTRA NASAL IZQUIERDA CONSTITUIDA POR UNA NEOFORMACION EXOFITICA ROJIZA DE 0.5 CM

  3. CASO CLINICO • SE PIENSA EN UNA RECIDIVA DE SU ENFERMEDAD Y SE LE TOMA NUEVA BIOPSIA DE PIEL, CULTIVO DE BIOPSIA PIEL.

  4. La dermatosis previa fue también coccidioidomicosis sistémica con compromiso cutáneo y pulmonar que respondió a itraconazol pero tuvo una recaída con su lesión intranasal,se le volvió administrar itraconazol 200 mg diarios por 1 año y actualmente el paciente lleva 3 años sin tener recidiva. • Paciente vive en un área endémica de coccidioidomicosis de Guatemala ( El Progreso) y su inmunosupresión favoreció la misma • Se tuvo una paciente trasplantada renal que visito un sitio de coccidioidomicosis y adquirió esta micosis en forma cutánea ulcerada.
