Action Team Goals and Objectives • Develop a comprehensive understanding of the Medicaid State Plan Amendment (SPA) process and requirements • Develop a SPA outline and work with DMHA/OMPP to influence and participate in the development of an Indiana SPA related the treatment of behaviorally based chronic health disorders • Assume a key leadership role in the development of the state’s healthcare home model • Act as the Action Team that pulls together work from other committees • Work to garner support from the new administration for a service delivery approach that improves health outcomes and reduces Medicaid costs 2-9-11 ICCMHC Meeting

Completed Deliverables • Developed a detailed action plan with team members and established due dates for each of the assigned goals/objectives • The ICCMHC met with IPHCA to discuss the role of respective organizations in the development of a healthcare home model • Action Team F met to establish the particulars of the Missouri and Rhode Island Healthcare Home models • Action Team F determined a draft approach related to the healthcare home information required as a part of the SPA development process • Ensured both Rhode Island and Missouri were able to present at February Council meeting 2-9-11 ICCMHC Meeting

Feedback Requested • Do you believe the ICCMHC/BHMI should move forward with healthcare home proposal with/without support of FQHC’s or other impacted organizations? • If the payment methodology that we develop is not supported by OMPP, how should the SPA Team re-evaluate the proposal? • How do you perceive the role of the ICCMCH/BHMI in the development of the SPA versus recommending elements of the SPA to OMPP? • Do you believe the healthcare home model should only modify current elements of the MRO program or completely revise the current program in favor of this approach? 2-9-11 ICCMHC Meeting
