  3. Our Mission To Join forces across all disciplines and establish pro active discharge planning ensuring patients are discharged safely and in a timely manner

  4. How are we doing today ? What staff said! • Better and early discharge planning, • Manage patient expectations, being realistic • Improve discharge process, quick response from social services, • Perceived delay in TTHs prescribing and dispensing • Need to discharge patients as doctors see them – not at end of ward round • Struggle to get discharge summary completed • Discharged patients to have access to ward rather than readmission

  6. Our Approach Engaging the right staff through a staff conversation Staff conversation – April 18th 2013 Attended by ward sisters, ward nurses, consultants, junior medical staff , pharmacist, occupational therapist, and physiotherapist

  7. 4 High Impact Actions • Ward Sisters / Charge Nurses across division work clinically on ward 8am-1pm, attending all medical ward rounds and then co-ordinating actions • Review role of patient status board specific to surgery, to include TTHs prescription, discharge summary. • Pilot on ward 14 - 4pm board review led ward sister with pharmacy, medical staff to ensure all actions completed • Establish satellite pharmacy unit on 1st floor (enable further reduction in turnaround time) space identified.

Benefits to Patients / Staff / Trust Daily review of all patients by ward sister / charge nurse supporting discharge as well as ensuring patients have the opportunity to ask any questions or voice concerns Organised discharge planning to enable discharge early morning which has positive impact on allocation of bed for elective and non elective admissions Improve use of discharge lounge Improved communication across all disciplines

  9. Summary and Next Steps The team have all enjoyed being part of this experience and were committed to ensure the high impact actions were achieved. Ward sister / charge nurses embraced the new way of working and have received positive feedback re leadership, visibility and engagement with colleagues as well as patients. Thank the staff on ward 14, who have all been so positive around how we can improve processes at ward level to enable a better patient experience. Now plan to roll out this process across the division Surgery specific patient status boards on order for all wards across division Satellite pharmacy to be completed
