  2. (NHS England 2016) Digital systems are the foundation upon which we will build a modern, efficient and responsive health service. Enabling information to flow between care providers within and beyond organisational boundaries, and between care providers and patients, is a key means by which we will achieve a safe, convenient and personalised health and care service

RDASH VisionTelehealth Strategy (2013) The future vision is that for those living within Doncaster with a Long term condition are offered the use of technology to empower and enable them to increase their confidence, promote independence and improve quality of life through supported self management of their health condition. This will be dependant on the patients/carers ability to manage the appropriate technology Embracing a technology approach to care delivery will aim to streamline care and improve quality and independence for the patient and family.

  4. What is Telehealth? “Telehealth is a broad term that refers to the use of technology to enable the remote delivery of healthcare or promotion of well-being”. University of Hull (2013)

Telemonitoring: The monitoring of patient vital signs and symptoms remotely Telecoaching: The use of CBT/Motivational interviewing to help patients achieve behaviour change over the phone Telesupport Helping the individual to self manage using education and support. Palliative telephone support Providing emotional and practical support to the patient, their family and carers using advanced Communication skills.

Advantages • Unnecessary check-up appointments can be avoided resulting in improved efficiency • Support to carers and family • Promotes patients self-management • Reduces face to face visits and reduces travel costs • Staff trained in motivational interviewing • Telehealth therefore has the ability to save money and improve patient lives and those of their carers

  8. Patient Story • Background :Mr B was referred to the Community Matron Service in 2011 after having two hospital admissions in two years with severe asthma. He felt acutely ill, confused and bewildered by what was happening and very low in mood . At 57 years old he was classed as unfit for work due to his complex medical history. • Initially supported by the Community Matron and then referred into the Telehealth service in 2012 for Telemonitoring of vital signs and symptoms. • Mr B was also referred for psychological therapies to come to terms with not being able to continue working and the impact this would have on his family life and emotional wellbeing • Mr B states he did not know about his own health, and lacked understanding on what was normal for him. He needed to make sense what was happing to him. He knew was unwell and felt as if he was going to die. • Mr B with the help of Telemonitoring and the Telehealth team now feels well educated and able to self manage his condition, recognise what's normal, and when to seek further medical advice. • Outcome more positive about his condition a sense of purpose by monitoring his vital signs and symptoms and acting on the findings. • To date no further hospital admission
